6月1日にASCOって学会で、膵癌（膵臓がん）の治療で新薬によって生存が半年から1年になったという発表がニュースになった。
「1年で死ぬって大したことないんじゃない？」って思うかもしれない。
ASCOってのは、がんの薬の治療の世界で最も権威があるといって良い学会。本物のすごい発表が行われる学会。

今回は膵癌で抗がん剤治療して効かなかった人を対象にした試験。新たに別の抗癌剤治療しても1年後に6人中5人死亡。それが今回の新薬を使えば、2人に1人以上生き残った。(18.2% VS 53.3%)
違うんだよ。専門家は違う視点でこれが見える。あのグラフを見ただけで驚愕する。
（グラフの2つの線の開きだけでもすごいんだけど、それ以外を解説するね。）
現在は平均して投与からまだ8.5カ月しか観察できていないのでこの結果。
本当は1年ではなくてもっと長く生きられそう。でもまだデータがない。
すごすぎる結果がでた時はいち早く世界に知らせないといけない。
世界では今日も膵癌で死んでる人がいる。古い治療を受けている人がいる。
本当にすごい結果がでた時は、5年間観察なんてせずにさっさと発表される。そして安全性に最大限配慮されつつも最速で世界中の治療が変化する。
あまりにも発想が新しい薬は、治らない病気を対象に行われる事が多い。
新薬の治療をして全員死んだらどうなる？もしも普通の治療をしていたら大半が治ってたとしたら？それこそ人体実験って言われる。
なので新しすぎる治療は、まずは治らない長生きできない癌に対して行われることが多い。効かなくても仕方ないって状況。膵癌とか神経膠腫とか悪性黒色腫とか。
ただしそれらの新しい治療の多くはあまり効果がなくて、やっぱりだめでしたねとなることが多い。でも今回は違った。劇的に効いた。
ニボルマブ（オプジーボ)って免疫治療は知ってる？京都大学の本庶先生がノーベル医学賞を受賞した新薬。
13年前のASCO 2013。
当時は治らないとされていた、1年で2割しか生存できないはずの悪性黒色腫が、1年で62％生存して世界が騒然とした。
（この次の日に、ニボルマブ＋イピリムマブって2つの薬を組み合わせたら、1年生存82％というさらにすごい数字の発表があって、世界が再び驚愕したんだけどね。）
ニボルマブはその後、他の癌、例えば肺癌にも効く事がわかった。現在は類似の薬がたくさん出ている。13年後の今、悪性黒色腫も肺癌も、「抗がん剤が効かない癌」から「抗がん剤が効く癌」に教科書は書き換わった。
診断時に手術ができない肺癌の患者さんは昔ならどれだけ頑張って治療しても長生きは目指せなかった。でも今なら、5年以上生きる事を目指せるようになった。(注釈：突っ込みがあったので弁明しておくと、Ⅲ期NSCLCのPCAIFIC trial 5y-over all survival > 40%を想定して書いています。)
今回、13年後のASCO 2026で、あの時と同じような成績でこの薬が発表された。
今回の薬は点滴不要、ただの飲み薬。副作用もきつくない。98%以上の人が中止せずに飲み続けられている。中止や減量の理由の多くは、口内炎や皮膚のトラブル。
治らないはずの膵癌、さらに抗がん剤が効かなかった症例という最悪の条件で、1年後に半分以上の症例が生き延びたのはものすごい希望。そして副作用も少ない。まだ8.5か月しか観察できていない。
今回は別に治療をした後って条件だったけど、最初からこの薬を使えば数年以上生き延びられるかもしれない。
他の薬と組み合わせたら、もっと効くかもしれない。
副作用のきつい抗癌剤治療を、この薬に置き換えられるかもしれない。
昨日は、1枚のスライドを見た瞬間に、世界中の癌のプロフェッショナルが理解して、今後を期待し妄想して、立ち上がって拍手を送った。
ASCOの共同発表者になったりJCOに共著で論文は出したことがある程度には癌治療に関わってきた人だけど、化学療法専門ではない。

「手術で数ヶ月待った」ってブコメを見かけたので乳がんで手術前に薬の治療をしたのかな？
今回の薬は基本的にはRAS変異のある癌がターゲット。もしも乳がんの場合は、乳がんでRAS変異は非常に少ないので対象外の可能性が高いけど将来的に適応が広がる可能性もあります。肺癌や大腸癌なら今後使えるようになる可能性はあります。毎年、色々新薬は出てくるのでこの薬はダメでも、他の新薬が使えるかもしれません。
ちなみに、将来万が一転移したとしても、どこか限定した場所に2-3箇所くらいまでの再発なら、放射線治療で再発した場所の癌をやっつける事は十分狙えます。万が一再発した時は一度放射線治療科へ相談してみる事を覚えておくと役立つかもしれません。

まー皆さまはCNNだからうんぬんとか言うのかもしれないけど、これに限らず、たとえばロシアやイスラエルへの反戦デモとかで、「世界中の国々でデモが起きています」と各国の映像をつなぎ合わせる報道とかよく見るんだよね。コメントの反応を見ていると、日本の政府を動かすというよりは、関係国の反戦派に勇気を与える効果の方が大きいような気がするね。
Ukrainian special forces carried out an incredible operation — they allegedly stole several fully fueled and armed combat aircraft directly from Russian airfields. HUR pilots stormed the parking areas, neutralized security, and flew the aircraft to Ukraine.
Impressive footage has already appeared: Russian Su-34s and bombers with Russian markings are seen landing at Ukrainian airfields under the escort of Ukrainian fighter jets.
Russian sources reported that, over time, 59 aircraft and helicopters were stolen, with some of them ending up in Ukraine. This was reported by the Russian outlet Izvestia, adding that some of the aircraft were later used in the war on Ukraine’s side against Russia itself. Because of this, the FSB has reportedly carried out a series of searches within Rosaviatsia as part of related criminal investigations.
According to Rosaviatsia, 36 out of the 59 aircraft disappeared in connection with being sold abroad. Of these, eight were sold to countries considered “unfriendly” by the aggressor. And these are only the confirmed cases — the ones the Kremlin allowed to be made public.
“Three Mi-8 helicopters later took part in the war on Ukraine’s side, while Il-76 aircraft transported cargo with their transponders turned off, concealing information about their movements,” Russian media also report.
It is also known that recently in Russia a Mi-8 helicopter simply disappeared during a training flight over a lake. There were three crew members on board.
Thus, the number of cases involving aircraft being transferred to Ukraine together with their crews is likely much higher than what Russian media are allowed to report.

故あって、初めてあいみょんの『マリーゴールド』をきちんと聴いた。
筆者はあまり日本のポップスに詳しいほうではない。一応聞くけど、まあコーネリアスだったり坂本慎太郎だったり、「サブカルクソ野郎」的なものをよく聴いているタイプだ。洋邦問わず聞くという自認ではあって、最近は個人的にロカビリー再評価ブームがきていてWild Roostersをよく聴いている。あとはモッズ、モッズが参照しているサザン・ソウルなんかを好んで聞くし、過去の音楽ではなく現在のものだとX-Over Jazz系をよく聴いている。Greg Speroとかね。そういえばKIRINJIの新譜よかったよな。リズム隊の「生感」がいままでで一番いいんじゃない？
で、そういう筆者が、故あって『マリーゴールド』をまじめに聴いたわけだ。聴く前の印象は「無難」「予定調和」「日本人の脆弱性を突いている」というような印象を持っていた。じっさい、コード進行、メロディ、アレンジにおいては、完全に「変」なところがない。こんなの「すでに刷り込まれたもの」を日本人に耳にぶち込んだら全員「好き〜」ってなるやつじゃん。という。まあ唯一アレンジにおいては逆再生？ ボリューム奏法？ がギミックとして仕込んであって、この手の保守的なポップスにおいてはちょっとおもしろいギミックだな、尖ってんじゃんと思った。思ったけれど、それ以外については全て「既視感マックス」の楽曲ではあると思ったし、それを覆すほどの強度のあるギミックではないよね。とはいえ、この「既視感マックス」をやるのってべつにそう簡単なことではないし、それをやるのはすごいんですよ。すごいんだけど、冒険心のない、「もうみんなが知ってて警戒心を持たなくてもよくてパッと入ってくる」という意味でたいへんに「日本の音楽文化で育ってきた人たちの心にスッと入っていく」脆弱性を突いたつくりの曲という意味では意外性やひっかかりが全くない曲だな、というのは、聴く前に持っていた印象とあまりかわりがなかったんだよね。
けど、一箇所だけ「え、まじ？」ってなって引っかかった部分があって、それが「むーぎわらの〜、ぼうしのきーみがゆれたマリーゴールドにに、てる」の部分。「に、てる」ってそこでブレス吸うんか！？ おなじメロディーのところでは全部こういう「文節の切断」が起こっている。こういう、文節をブレスやフレーズを切っちゃうやりかたっていうのはまあ今までもやられなかったわけではない。宇多田ヒカルの「な、なかいめのべ、ルで受話器を取った君」もだいぶへんなところで切ってる。けどこういうのってあんまり「伝統的で既視感マックスで保守的な"歌謡曲"」でやることってあんまなくない？ そこにぼくはちょっとアンバランスさを感じて「ひっかかり」を得たんだよね。
ポップスにおいて、この「ひっかかり」ってのはかなり大事だと思っていて、「なじみのある感じ」と「ひっかかり」が両方あって初めて「ポップスとしておもしろい曲になる」と思うんだよね。何度も言うけど、べつに「文節を切っちゃう歌い方」ってこれがはじめてじゃなくて、いろんなところでやられているけど、これだけ曲もコードもアレンジも保守的な「歌謡曲」で、こういうことをやられているのは結構大きな違和感としてかなり仕事していると思う。
で、これが意図的なのかどうかはわかんないんだけど、めちゃめちゃいい違和感じゃんこれ、とぼくは思ったわけ。この文節のぶったぎりがなければ、「凡庸な歌謡曲」で終わってもおかしくないところを、この文節ぶったぎりがそうさせていない。逆に、アレンジ、コード進行、メロディどこかに「斬新さ」があったらこの文節ぶったぎりは「あ〜はい、そういうジャンルね」で回収された気がする。そういう意味で、「基本全部めちゃめちゃ保守的」に、一箇所だけ「保守的じゃないやん！ という違和感」を忍び込ませている構造はかなり「新しいしキャッチー」を成立させているな〜って思ったんだよな。
最初に書いた通りぼくは日本のポップスあんまり詳しくないから、的外れなこと言ってるかもしれない。けどぼくは「あいみょんって要するに懐メロの焼き直しでしょ」って思っていたことを懺悔したい気持ちになったよ。という記録

I've been an IT engineer in the states for over 20 years and do have a friend who hopped jobs 3 times in 3 years, 2 of them less than a year. So I think I know a thing or two about how IT jobs work in here. Like I said, a tenure that is generally considered good is around 3-5 years. Think about it, what can you achieve in mere 8 months that you can leverage on in the interviews for the next job?

子供が失敗すると激しく怒ったり落ち込んだりするのは、自分のナルシシズムが傷つくから。
日本でよく言われる俗語的な表現だと「教育ママ／パパ」の極端な形
と言われますが、心理学の正式な用語では上記の①～④が一番近いです。特に一番ピッタリくる単語を挙げるなら
→ 「過剰同一化（over-identification）」
または
→ 「子供を通じた代理達成（vicarious achievement）」 この2つが最もよく使われます。このような親は、子供が不登校や燃え尽き症候群（バーンアウト）になるリスクが非常に高いことも、臨床心理学ではよく指摘されています。
