RETURN DEMONSTRATION: URINARY CATHETERIZATION

PERINEAL CARE:

1. Introduce yourself.
2. Greet the patient and explain the procedure.
3. Change top sheet into bath blanket and position it properly (diamond).
4. Position the patient in a lateral position. Bend the patient's one leg and pull the patient
towards you.
5. Get the bed pan. Warm it by using the newspaper nga naka cover sa bed pan.
6. Put the bed pan sa patient's butt area while you turn back the patient into its original
supine position. Make sure that the bed pan is placed properly.
7. Naka bend ang both knees sa patient (dorsal recumbent position) that allows the pelvic
area to be exposed.
8. Drape the patient. Tuck side corners of the bath blanket around patient's legs. Kutob sa
above knee area ang placement or piko sa bath blanket leaving the middle area
(perineal area) covered.
9. I expose dayon and perineal area nga covered sa bath blanket by folding it up unto the
patient's abdomen.
10. Attach the drawing (naa ray tape si maam).
11. Tell the patient that you're about to do the perineal care.
12. Wear clean gloves.
13. Get the pitcher w/ water then tell the patient that it's a bit cold so that ma aware siya.
14. Pour the water at the middle part or sa vaginal area including the rectal area.
15. Soap solution: Get one cherry ball by using the picking forceps and receive it on the
other hand with the use of the dressing forceps.
16. Follow the pattern for cleansing perineum (6 cherry balls would be used).
17. Get the pitcher w/ water then pour at the middle area.
18. Betadine Solution: Same technique used during the soap solution. Get one cherry ball by
using the picking forceps and receive it on the other hand with the use of the dressing
forceps.
19. Follow the pattern for cleansing perineum (6 cherry balls would be used).
20. Note: Dili na mag wash sa perineum using water after cleansing with betadine solution.
21. Cover the exposed area with bath blanket.
22. Remove the gloves.

CATHETERIZATION:
o Get the kit.

o Outer: use bare fingers to uncover it.
o Inner: Aseptic technique. Use picking forceps.
o Drop the rest of the materials that are not found in the kit using aseptic
technique to maintain the sterility of the area such as:
 Foley and straight catheters
 gauze pads
 syringe (if walay attached needle ang syringe then drop the g19 needle)
 Urobag
 Sterile gloves
o Open the jelly sachet and put its content above the gauze pads
o Put the urine receptacle and sterile water OUTSIDE the sterile area.
o Expose the covered perineum.
o Wear the sterile gloves using aseptic technique.
o Get the eye towel(sterile) from the kit and attach it anterior sa katong drawing.
Be careful not to touch non-sterile surfaces since you're already sterile.
INSERTING STRAIGHT CATHETER:
1. Get the straight catheter. Roll it around the dominant hand.

2. Lubricate the straight catheter w/ the jelly solution nga na place sa gauze pads
(2-3 inches).
3. Get the kidney basin(sterile) from the kit and place it in front of the perineal
area.
4. Spread the eye towel using the non-dominant hand then insert the straight
catheter using the dominant hand while instructing the patient to inhale deeply.
5. Buhi'an ang gi roll nga catheter then i sawd ang tumoy sa catheter padung sa
urine receptacle while getting the amount needed for laboratory.
6. Then i sawd ang tumoy sa catheter sa kidney basin while you set aside the urine
receptacle.
7. Kink then fold the catheter around the dominant hand and withdraw it. Done!!
INSERTING FOLEY CATHETER:
1. For foley catheter: Assuming naka new gloves naka.

2. Get the syringe. Aspirate 10cc of air and inject it into the sterile water. Get the
same amount sterile water.
3. Hold the balloon port or the orange part only of the foley catheter(para dili ka
ma prick) using your two fingers.
4. Inject the sterile water sa lungag.
5. Check if there are any leaks. If there's none then you can now deflate the balloon
by withdrawing the sterile water from the catheter.
6. Note: do not recap the needle
7. Get the urobag and check if it's close.
8. Connect together the urine drainage port of the foley catheter and the urobag.
9. Kink the foley catheter then apply lubricant.
10. Retract the eye towel.
11. Instruct the patient to inhale deeply then insert the foley catheter.
12. Get the syringe with needle then inject the sterile water inside the balloon port
of the foley catheter.
13. Check if it's already secured by checking if there's resistance sa balloon.
14. Remove the eye towel.
 For males: Secure the catheter tubing to the upper thigh/abdomen using
a tape.
 For females: Secure the catheter tubing to the inner thigh using a tape.
15. Place the urobag at the side of the bed and hang it there.
16. For removal: tangtangon ang tape then aspirate the sterile water from the
catheter.
17. Kink the catheter.
18. Instruct the patient to inhale deeply then discard or withdraw the catheter.
