“What you should be concerning yourself with is that Leone gaming house up in Saint Mark's. The Sindaccos are going nuts about it. I heard they're gonna blow the place off the map.”
Toni goes to Paulie's Revue Bar looking for more work from JD, who is entertaining some '"special" clients. He warns Toni about the safety of the Leone Crime Family-controlled casino in Saint Mark's. Toni leaves and drives to the casino, where he receives a phone call from JD. JD warns him of the Sindacco Family's plans to destroy the casino.
A group of Sindacco Family gang members arrive in a Sindacco Argento, with one of the gang members attempting to rig a bomb outside of the casino. Toni kills and stops the gang members, but is met by a second wave, who also arrive in another Sindacco Argento but wield MAC-10s. Toni once again kills and stops the Sindacco family gang members. A third group in a Patriot arrive with Micro-SMG's in hand, but Toni again manages to fend them off.
After killing all three groups, some more Sindaccos arrive in a Yankee rigged with explosives and park it outside the casino. When Toni attempts to move it, he finds that the Sindaccos have sabotaged it to remain in first gear, severely limiting its speed. Toni eventually manages to move it a safe distance from the casino before it explodes.
Hey, Toni, I can't come out right now, I'm busy, you know?
Toni:
Why? What's going on in there? And what the hell is that smell?
JD:
Look, Toni, we're catering for some real, uh... specialists today. This stuff is internet gold-dust! What you should be concerning yourself with is that Leone gaming house up in Saint Mark's. The Sindaccos are going nuts about it. I heard they're gonna blow the place off the map.
Toni? It's JD. Look, I just heard there's a bunch of Sindacco goons coming your way, and they're packing, big time. Watch yourself!
Toni:
Thanks, JD! Appreciate it.
Toni goes to deal with the Sindacco members that want to blow up the casino, with the first two groups of Sindaccos arriving in Sindacco Argentos and the third group arriving in a Patriot. Another Sindacco member attempts to blow up the casino by driving a bomb-rigged Yankee up to it, but Toni takes care of the bomb.

Get the truck away from the Casino before it blows! If the truck is not destroyed before reaching the casino, the Sindaccos will arm the timed bomb and Toni must drive it away from the casino before the time runs out.
The Casino is safe. Get out of there! Taking the truck to a safe distance from the casino.
The truck is destroyed.

This mission may be a homage to the film The Long Good Friday, which involves gang members planting bombs in various places owned by the film's protagonist, including his casino (the exterior of which looks somewhat similar to the Leone casino).
