Report an Incident to the University

The Equal Opportunity and Compliance Office (EOC) handles student, employee, faculty and visitor reports of discrimination and harassment based on age, color, disability, genetic information, national origin, race, religion, sex (including gender, gender expression, or gender identity), sexual orientation, and veteran status. This includes reports of sexual misconduct, including sexual harassment and sexual violence. Our office also handles reports of relationship violence and stalking and reports of workplace violence.
Once you submit this form, you may be contacted by someone from the University’s Equal Opportunity and Compliance Office including a trained Report and Response Manager. Regardless of the nature of your report, the University will ensure that your report is forwarded to the office with responsibility for responding, as appropriate, based on the information you have provided.
Please note that reporting an incident without disclosing your name (i.e., anonymously) may prevent the University from being able to provide you with supportive measures (such as academic accommodations, no-contact orders, safe housing options, change of work schedule) because staff will not be able to contact you. Reporting anonymously may also impact the University’s ability to respond and/or pursue disciplinary action.
