1 lesson plan/DLL from Quarter 1 OR Quarter 2 showing that achievable and appropriate learning
outcomes are ALL aligned with the learning competencies as demonstrated in any part of the lesson,
and the outcomes led to the attainment of the next related competency

List of identified least/most mastered skills, with accomplishment report for


remedial/enhancement activities, from Quarters 1 AND 2

Any proof, from Quarter 1 AND Quarter 2, showing sustained engagement through regular
communication of learners' needs, progress and achievement to key stakeholders, including
parents/guardians

Samples:
> Home visit forms, 1 in Quarter 1 and 1 in Quarter 2, showing agenda of visit,
observations/findings, and further agreements
> Parent-teacher conference attendance log and minutes of the meeting, 1 in Quarter 1 and 1 in
Quarter 2, showing conference agenda and further agreements

Any proof, from Quarter 1 AND Quarter 2, showing participation in any professional
network/activity that requires output and proof of implementation within the school to share
knowledge and to enhance practice

Samples:
> Certificate of participation in a training/seminar/workshop/LAC Session, 1 in Quarter 1 and 1 in
Quarter 2, which requires output and proof of implementation
> Certificate of completion in a course/training, 1 in Quarter 1 and 1 in Quarter 2, which requires
output and proof of implementation

1. Certification from the ICT Coordinator/School Head/Focal Person in-charge of e-SAT, stating that
it was done during the start of the SY
2. IPCRF-Development Plan based on e-SAT results, accomplished and submitted immediately after
accomplishing the e-SAT
Any proof of participation in other related works/activities that contribute to the teaching-
learning process, 1 in Quarter 1 AND 1 in Quarter 2, with annotation detailing the achieved
positive contribution to the teaching-learning process, and one of which contributed to the
teaching-learning process beyond the school/CLC

Samples:
> Certificate of committee involvement, annotated with the achieved positive contribution, in the
district/division level in Quarter 1 and in Quarter 2
> Certificate of participation as a research presenter in a forum/conference, annotated with the
achieved positive contribution, in the district/division/regional/national level in Quarter 1 and in
Quarter 2

positive contribution to the teaching-learning process, and one of which contributed to the

Mid-Year Review)
