Deciding to take stock, Cletus Kasady comments that his former symbiote is currently off rampaging across the Multiverse, lamenting not having joined its pursuit of godhood. Deciding to stay the course on Earth, Cletus recaps using the Iron Man armor he'd torn off Tony Stark to hack into his communications network and convert everyone with a Stark-brand device into a bloodthirsty berserker. However, despite being more powerful than ever, there's still a few persistent thorns in his side: Normie Osborn, whose symbiote -- made up of leftover pieces of Carnage -- can absorb others; and Miles Morales, whose bioelectric powers are strong enough to hurt him. Concluding, Cletus says that he's now talking to himself in an empty lab because he doesn't know how to handle being alone and whenever he starts to feel small he always multiples himself. As Cletus notes that whether it's a symbiote dragon, a suit of Iron Man armor, or a hatchet from a hardware store, he always uses his tools to turn everything into a mirror -- something that makes him predictable and therefore vulnerable -- Extreme Carnage asks if his copy is proposing a change in tactics. Refuting this, Cletus says he's going to double down and and has been thinking too small, that instead of trying to reach millions of people they should be reaching trillions. Gesturing to the scattered pieces of old Iron Man armor around the lab, Cletus says he should use them to build a bigger mirror. Pinging a trio of IP addresses, Extreme Carnage says he knows just what to do, the Cletus copy saying that he's only ever needed himself as he's reabsorbed.

In the streets below, Miles and Normie struggle to hold back the berserk civilians in Cletus' thrall. As Normie says he wants to try something, Miles tells him to hurry up as the Stark Sentinels prepare to commence "sterilization" of the affected people. Grabbing a man, Normie successfully uses Rascal to free him from Cletus' control.

In a Midtown police station, Kenneth Neely -- wrapped in a straitjacket -- rambles about how he once thought he was an artist but that he was just a fool and that Carnage and Cletus both were right to find him contemptible. As he devolves into broken laughter, Agent Julia Gao comments that everything Cletus Kasady touches ends up being destroyed. Scoffing, Feilong dismissively remarks that the symbiotes, like mutantkind, are just another disease to be cured. As Agent Gao asks if he's come up with said cure, Feilong pulls out a vial and says he developed a nanotech serum that will purge Extreme Carnage from Stark's network, and that the rampaging mob is under the thrall of a sonic emission that his people are already working to counteract. Declaring his intent to save the world from his hotel room, Feilong leaves and orders the Stark Sentinels to engage Extreme Carnage directly.

As the two giant robots take off, Normie says that he needs the Cape Killers to get him to Cletus if they want to shut down the attack with minimal casualties. Taskmaster scoffs at this, saying that Normie's insane if he thinks he could just absorb Cletus Kasady given he's already on the ropes. Miles agrees with Taskmaster, asking Normie if he thinks he can handle it, Rascal briefly goes Goblin Mode as Normie shouts for them to shut up and just get him to Cletus. Feilong monitoring from his hotel room, the Stark Sentinels breach the symbiote-encased Stark Unlimited HQ; Extreme Carnage expressing incredulous fury that they sent robots to fight him. Blasting Extreme Carnage with their chest-mounted Unibeams, the Stark Sentinels begin uploading the payload. Sneering that there's no fun in destroying robots, Carnage dismembers and crushes one of the Stark Sentinels, hacking into it to mock Feilong. Outraged and horrified, Feilong activates the compromised Stark Sentinel's self-destruct.

Looking out over the burning city, Agent Gao says she doesn't trust Feilong's mechs to be able to take down Extreme Carnage, greenlighting the Red Goblin's proposal. As the Scorpion warns her that absorbing the Extreme Carnage symbiote is taking a toll on the Red Goblin's mind, Agent Gao says they can worry about that after Cletus is stopped. Barking orders, Scorpion tells Electro to clear a path without killing the infected civilians, and Hightail to evacuate those who've been freed from Cletus' control. As Spider-Man and the Scorpion steady him, Normie asks if they'll pull him out if he loses it, Miles saying that they will and Gargan saying that they probably won't; Taskmaster complaining about being left behind to hold the line. As Spider-Man, Red Goblin, and the Scorpion begin climbing the side of the Stark Unlimited HQ, Red Goblin alerts the others to the horde of berserk civilians climbing after them.

Up above, Extreme Carnage attempts to crush the second Stark Sentinel, noting that it's sturdier than its companion. Just as Extreme Carnage crushes it, the Stark Sentinel finishes uploading Feilong's nanite countermeasure, shutting Cletus out of Stark's network. As Feilong gloats, Extreme Carnage furiously declares his intent to hunt him down and make him die a slow, tortuous death. Spider-Man sends Scorpion in as a distraction, Extreme Carnage mocking him for thinking he could take him down. As Extreme Carnage snares the Scorpion, Red Goblin begins to absorb him, interfacing with Cletus' mind. Horrified, Normie says it's like staring into a bottomless ocean of hatred and bloodlust. Normie attempts to back out, but Cletus senses his presence and mind-bombs him, leaving him on the brink of unconsciousness. Breaking free of Extreme Carnage's tentacles, the Scorpion runs as Miles picks up Normie; Extreme Carnage snarling that he's not going to let them leave after ruining his fun. Undergoing a metamorphosis into an armored draconic form, Extreme Carnage attempts to devour the three as they leap out of the building.

Watching a livestream of Extreme Carnage bursting out of the building, @gooeyguy430, @carnijftw, and @symbiGOAT212 cheer Cletus on before their doorbells ring. Answering their doors, the three find symbiote-infused suits of Iron Man armor standing outside, Cletus speaking through them to say he's chosen to reward their loyalty by making them his missionaries.

In New Jersey, Tony Stark seethes as he watches footage of Extreme Carnage's rampage. Suiting up in a knight-themed suit of Iron Man armor, Tony declares he's going to end Cletus' reign of terror once and for all.
