Gordon's functional health patterns is a
method devised by Marjory Gordons to be used by nurses in the nursing process to provide a more comprehensive nursing assessment of the patient. FUNCTIONAL HEALTH ASSESSMENT TOOLS
1. Health Perception & Health Management
Asking the patient about his life style, health and awareness ..
How would you describe your health overall, at this time..?
What is the reason for this health care visit, expectation? Are you aware of any risk factors for disease that you have ? 2. NUTRITION & METABOLISM Ask the patient specific question about foods , weight and any skin issues: Describe what you usually eat, drink . Breakfast? Lunch ? Dinner ? Snacks? Describe your appetite ? Any problems affect your appetite ( nausea , vomiting ) ? Have you noticed problems with your skin ( dryness , lesions and itching ) ? Do your wounds heal quickly ? 3. ELIMINATION Ask the patient about urination and bowl elimination pattern and any assistive devices : • How many times a day do you urinate ? Any problem while Urinating (pain, dribbling, retention, frequency) ? • Describe your normal bowl elimination patter, including time and frequency , color , consistency ? 4. ACTIVITY & EXERCISE Ask the patient the daily activity he is doing and whether he need assistance or not : Describe your activity ? Do you exercise ? If yes, describe . Which leisure activities do you enjoy ? Do you experience any problem while doing activity ( SOB , weakness , fatigue , cough , chest pain …? If so, describe . To what extent do you require assistance while doing activity? 5. SLEEP & REST ASKTHE PATIENT ABOUT SLEEPING PATTERN, TIME, WAKE UP TIME, PROBLEM ASSOCIATED WITH SLEEPING . 6. COGNITIVE & PERCEPTION Ask the patient about visual , hearing , reading , writing and speaking problems.
7. Self –Perception & Self – Concept
Ask the patient about himself, concerns and feelings
8. Roles & Relationships
Ask the patient about relationship , family , groups , friends conflicts resolve….. 9. SEXUALITY & REPRODUCTION Ask the patient about activity , protection from transmitted infection , changes , problems , pregnancy and menstruation.
10. Coping & Stress Tolerance
Major Changes in life , how to handle problems , mood status and if any medication used helping him to relax.
11. Values & Belief
Ask the patient about plans or goals of future , importance of the religion and any personal values , belief maybe compromised. SUMMARY Helping you to identify the patient fundamental health and wellness systems are out of balance and what steps to bring them back into balance.
