Guide in accomplishing the PMCF

1. Date: Indicates when the PMCF was accomplished.


2. Critical Incidence Description: Narrates either or both positive or negative instance/s related to
the performance of the coachee’s KRAs.
3. Output: This refers to the immediate result/s of the incident.
4. Impact on Job/Action Plan: For positive incidents, the Impact on Job due to the incident will be
cited. For negative incidents, an impact and Action Plan shall be indicated.
