1. In relation to Division Memorandum No.12, s. 2021, this district announces the conduct of District
Evaluation viz a viz Implementation of the Seal of Exemplary Leadership and School Governance (ELSG) on
April 26-30, 2021 which aims to assess performance of teachers and school heads to mirror quality
performance and redirecting governance to foster transformational leadership for public Elementary and
secondary schools for SY 2020-2021.

2. All Schools shall undergo the Seal of ELSG and Classroom Evaluation.

3. Enclosed in this memorandum are the guidelines for the Seal of ELSG and District Evaluation Tool
for classroom under the new normal.

4. Expenses relative to the conduct of this activity are chargeable against local funds/MOOE subject to
the usual accounting and auditing rules and regulations.

GUIDELINES FOR THE SEAL OF EXEMPLARY LEADERSHIP AND SCHOOL
GOVERNANCE (ELSG) AND DISTRICT EVALUATION TOOL

I. Rationale:
The Seal of Exemplary Leadership and School Governance (ELSG) was created in order to establish
a unified and standard evaluation procedure for all programs and projects in support to the attainment of the
Division targets for three consecutive years. All elementary and secondary schools shall undergo the Seal of
ELSG evaluation. The Seal of ELSG will be implemented for three consecutive years starting SY 2020-2021.
A set of guidelines will be issued with some indicators added each thus having an increasing level of
compliance until the last year is reached.
For SY 2020-2021, the Seal of ELSG shall focus on important indicators adopting the new normal
situation and in preparation for possible resumption of face-to-face instruction.

II. Scope:

There will be three (3) phases of Evaluation. The first phase will be conducted by the district. The
second phase will be the Municipal level and the third phase will be conducted by the division level.
