Bone Fractures Types Nursing Interventions, Treatment, Signs and Symptoms NCLEX
Bone fracture types nursing review for the NCLEX exam that covers treatments, signs and symptoms, causes, and nursing interventions.
A bone fracture is a break or crack in a bone. There are several types of bone fractures. They include the following:
-Complete Fracture: the fracture completely separates the bone in two.
-Incomplete Fracture: the fracture does NOT break the bone all the way through.
-Greenstick: one side of the bone is bent while the other is broken…incomplete type of fracture (most common in pediatric patients)
-Comminuted: the bone is broken into many fragments (3 or more).
-Transverse: the fracture is straight across the bone shaft.
-Oblique: the fracture is slanted across the bone shaft.
-Spiral: the fracture twists around the bone shaft (from a twisting injury...

Complication of a bone fracture can include infection (osteomyelitis), compartment syndrome, fat embolism (most common with long bone fractures) etc.
Nursing interventions include to assess the 6 P's (pain, pallor, paresthesia, paralysis, poikilothermia, pulselessness), immobilization of the affected area, stopping the bleed, cast and traction care, monitoring for compartment syndrome and fat embolism etc.

Compartment syndrome nursing NCLEX review practice question on the nursing interventions and neurovascular assessment for this condition.
Compartment syndrome occurs when too much pressure is exerted within the muscle compartments found within the fascia.
This can occur when there is hemorrhaging (bleeding) or swelling present after an injury, like with a bone fracture (or with external factors like a cast being too tight or traction). All this can increase the pressure within the compartments. As the pressure builds, this will cut off the blood supply and nerve function to this muscle. If not corrected within 6 hours, the damage is permanent.
Remember from anatomy and physiology that in the leg and arm there are individual compartments grouped together (but separated from one another) that contain bone, muscle, nerves, and vessels. Each compartment usually has its own muscle, nerve, and vessel supply. Fascia is what keeps all these structures in place and separated.
The important thing to remember about fascia when talking about compartment syndrome is that is does NOT expand when pressure increases within a compartment (so there will be no relief within the compartment from the fascia).
Instead the pressure stays within the compartment and causes blood vessel and nerve function to become compromised (diminished). So, ischemia is going to occur to the muscle and distal extremity to the fracture.
Nursing interventions include: keeping the extremity AT HEART level (NOT below....remember you want to maintain arterial pressure and elevating it above heart level will cause more ischemia), loosen and remove restrictive items, notify the physician, perform neurovascular checks (6 P's), prepare the patient for possible bivalvement of the cast, reduction of weight in the traction, or in severe cases fasciotomy.

Osteoclast consume bone minerals (calcium and phosphorus) within the spongy bone when levels of calcium are too low in the blood stream. This will increase the levels in the blood.
Osteoblasts build up the matrix of the spongy bone by using the excessive amount of calcium and phosphorus present in the bone. The activity of osteoclasts and osteoblasts are controlled by various hormones such as: Parathyroid hormone (PTH), Calcitonin (produced by the thyroid gland), growth hormone, estrogen, and testosterone.
Risk factors for developing osteoporosis include: low calcium and vitamin D intake, older age, smoking, regular alcohol usage, sedentary lifestyle or immobility, genetics, underweight (BMI less than 19), thin, small frame, medications (glucocorticoids and anticonvulsants).
Sign and symptoms of osteoporosis include: fractures (hip, wrist, spine), rounding hump of the upper back (Dowager's Hump), asymptomatic, loss of inches of height from young adulthood height (2-3 inches), lower back or neck pain.
A DEXA scan is commonly used to assess the presence of osteoporosis. This measures the bone's density with x-ray imaging.
Osteoporosis treatment includes: further prevention with weight-bearing exercises, smoking cessation, healthy diet that is rich in calcium and vitamin D, low alcohol intake, medications (bisphosphonates etc.)
Nursing care for osteoporosis includes: safety (preventing further fractures), education on medication and prevention, assessing for signs and symptoms/risk factors, and monitoring for side effects of medications.
