As implied, the ICBM is meant to assume the appearance of a mobile intercontinental ballistic missile (ICBM) launcher, complete with an unspecified ICBM missile loaded on the vehicle. The launcher itself appears to be based on the body of the game's Fire Truck, with only the cab repainted green.

Being a large vehicle, the ICBM is heavier than even the Fire Truck but lighter than the Royal Pain, with a substantially low top speed, average acceleration, and poor brakes, grip and handling. Despite possessing a base export of value of £900, the ICBM cannot be exported as it is too large.
Although not as tough as the Royal Pain, it can take more damage than most large vehicles in the game, even (luckily) being able to take quite a lot of gunfire.

- The ICBM is most prominently featured in a mission in GTA London 1969 where one ICBM is successfully stolen by Hans Nemesis until the protagonist, under Endeavour Chambers' orders, manages to find out the ICBM codes from two cars around London and stops the ICBM from launching its payload. It can blow up London if certain time requirements are not met in this mission.

- The ICBM is based on a nuclear missile used during the Cold War.
- The "GTA Spy Theme" plays when entering the ICBM. This is is same music which plays when entering the Jug Swinger owned by secret agent Endeavour Chambers.
- After deactivating the ICBM, the mission will complete, and the spymasters will call the protagonist a "hero among men and a stud among the ladies. Now go out and cause havoc, have as much fun as you want!" The player can then use the ICBM as they please. Be careful - it can be destroyed, and although it will not create a nuclear explosion (since the missile is now disarmed), the blast radius will still be much larger than a normal car explosion. This causes all vehicles on screen to explode and it likely the player will be killed if standing too close to a vehicle when this happens.
