PARENT CONSENT AND WAIVER FORM


I ___________________________, grant my permission for my son/daughter, __________________________, to participate in
the Leadership Training Seminar entitled: “Leadership Development: Unleashing Your Inner Potential” on September 21
and 28, 2019 (Saturday) from 8:00AM – 5:00PM at Bagong Barrio National High School – Lobby.

This activity provides students with the opportunity to:


 Learn the character traits of a leader and appreciate their own uniqueness;
 Learn to act out of humanitarian values;
 Gain a greater understanding of their own personal identities and how their identities shape their leadership and
followership;
 Learn how to use their minds, their hearts, their voices, and their ears in order to constructively engage and
collaborate to others; and
 Understand and be able to utilize a process for decision-making

I, as parent and/or legal guardian, remain legally responsible for any personal actions taken by the above named participants.
I agree on behalf of myself, my child named herein, or our heirs, successors, and assigns, to hold harmless and defend Bagong
Barrio National High School, its faculty, administrators or representatives associated with the event, from any and all actions,
claims, demands, damages, costs, expenses and all consequential damage arising from or in connection with my child attending
the said activity.
Im signing the Parental Consent and Waiver, I am not relying on any oral or written representation or statements made by school
and its faculty, administrators or representatives associated with the event, to induce me to permit my child to take the seminar,
other than those set out in this consent waiver.
I am 18 years of age and more and have read and understand the terms of this consent and waiver, and recognize that it is
binding upon me, my heirs, executors and administrators.
