Challenge: Full Tilt Affair
Tethered in pairs, castaways must race across a series of obstacles while untying and grabbing a sandbag along the way. They must then attempt to land the sandbag on a narrow disc. The first pair to land their sandbag on their disc wins.
Reward: A feast with their loved ones
Winners: Orange Team (Janet Carbin and Tommy Sheehan) (shared with Dan Spilo and Lauren Beck)
Challenge: Ballin a Jack
Castaways must stand on a small block while stabilizing a ball against an overhanging piece of wood. If at any point they lose their concentration or their balance, the ball will drop and they are then out of the challenge. The last person left standing wins.
Winner: Lauren Beck

- The title of this episode is said by Dean Kowalski when he was discussing Noura Salman's conversation with him about their plan to blindside Tommy Sheehan.
- Host Jeff Probst erroneously stated that Dan Spilo's son Ryan is the youngest person to appear as a loved one during the Family Visit, doing so at the age of 13. However, Shane Powers' son Boston was 12 years old when he appeared during the family visit in Panama.[2]
- With Elaine Stott finding a Hidden Immunity Idol, this marks the tenth idol in play this season, breaking the previous record of nine in Heroes vs. Healers vs. Hustlers.
- As of this episode, Karishma Patel has broken Laura Morett's record for most elimination votes cast against a player in one season.
- In addition, Dean tied Jane Bright and Terry Deitz's record for voting for the same person the most amount of times in a season, having voted for Karishma five times.
- Dean also has the record for casting elimination votes for the same person at the most Tribal Councils, as one of Jane's votes for Marty Piombo in Nicaragua was on a revote, and one of Terry's votes for Aras Baskauskas in Panama was at the Final Tribal Council.
