A form was required to be signed by a third-year student's parent or guardian in order for that student to take part on the weekend trips to Hogsmeade Village, an all-wizarding village near Hogwarts School of Witchcraft and Wizardry.[1] Harry Potter was unable to get Vernon Dursley to sign his copy of this form[2] but eventually received a separate signed note of permission from his godfather Sirius Black, which was considered "good enough for Dumbledore".[3]
It was possible for a Head of House to deny a student permission to enter Hogsmeade even if they had a permission form. This was the case with Jacob's sibling in 1986. Their Head of House did not allow them to go to Hogsmeade due to breaking the rules but allowed them to go after they studied hard.[4]

Behind the scenes[]
- "A signed Hogsmeade permission form" was one out of the thirty-two options generated by an old feature[5] on Pottermore, "Wonderful wizarding world happiness generator", which was about things that could cast away the Blue Monday blues.[6]
- The description was "Harry had a hard time getting into Hogsmeade thanks to a typically hostile Uncle Vernon, but here's a signed form we made just for you. Meet you at the Hog's Head!".[7]
