In comparison to Forky's simplistic design, Karen Beverly is extremely detailed; her body is a plastic white knife, with eyelashes and freckles drawn on her face with black marker. She has two different sized googly eyes and a red wax mouth. She has yellow yarn hair glued on the top with a purple sequin flower jewel. She also has a blue wax necklace and a green dress cut from construction paper, with a purple wave foam sticker as a hem. In the corner of the dress, she has two purple felt star stickers and a gold sequin. She also has green pipe cleaner arms, a clay base, and two ends of broken popsicle stick for legs, with Bonnie's name written between at the bottom, written in red marker.

Toy Story 4[]
In the final sequence of the mid credits scenes, Bonnie drops off her backpack into her bedroom after her first day of first grade. Jessie climbs out of the backpack, with the toys feeling ecstatic for her to tell how the first day of school went. Jessie says that it went great, and that Bonnie once again "made" a new friend. She coaxes and pulls out Karen Beverly, who is a plastic knife made from trash that has come to life. The toys clear the way for Forky, who immediately falls in love with Karen. As they waddle towards one another, Forky introduces himself and the group as "unique and beautiful" toys. And then, he says that he'll explain everything. But when Karen asks how she's alive, Forky simply says that he doesn't know.

- According to the audio commentary, her name was confirmed by Josh Cooley and Mark Nielsen during the credits. They thought that they should give her a unique name in comparison to how many characters have names ending with an "E" sound (Woody, Jessie, Slinky, Trixie, Dolly, Forky, etc). Josh suggested using the name "Karen", named after Mark Nielsen's sister, followed by "Beverly", also Mark's sister, which is used as a suggestion for the character name.[1]
- The name "Knifey" is colloquially used as an in-joke of many characters in the Toy Story franchise whose names end with a long "E" sound.
