Family Care Plan
the blueprint of the care that the nurse designs to systematically minimize or eliminate the identified health and family nursing problems through explicitly formulated outcomes of care (goals and objectives) and deliberately chosen set of interventions, resources and evaluation criteria, standards, methods and tools

focuses on actions which are designed to solve or minimize existing problem a product of a deliberate systematic process utilizes events in the past and what is happening in the present to determine patterns
projects the future scenario if the current situation is not corrected

based upon identified health and nursing problems means to an end, not an end in itself nursing care planning is a continuous process, not a one-shot-deal

Prioritizing Health Problems
Criteria:
Nature of the condition or problem categorized into wellness state/potential, health threat, health deficit and foreseeable crisis Modifiability of the condition or problem refers to the probability of success in enhancing the wellness states, improving the condition, minimizing, alleviating or totally eradicating the problem through intervention

Criteria:
Preventive Potential refers to the nature and magnitude of future problems that can be minimized or totally prevented if intervention is done on the condition or problem under consideration

Salience refers to the familys perception and evaluation of the condition or problem in terms of seriousness and urgency of attention needed or family readiness
CRITERIA
1. Nature of the condition or problem scale --- wellness state health deficit health threat foreseeable crisis 2. Modifiability of the condition or problem scale --- easily modifiable partially modifiable not modifiable 3. Preventive potential scale --- high moderate low 4. Salience scale --- needing immediate attention not needing immediate attention not perceived as a problem or condition needing change 3 3 2 1

SCORING
Decide on a score for each criteria Divide the score by the highest possible score e and multiply by the weight Sum up the scores for all the criteria. The highest score is 5. *conditions/problems with total scores nearer 5 is taken priority

NATURE/CONDITION OF THE PROBLEM highest score is given according to the sense of urgency which may require immediate intervention

Current knowledge, technology, and interventions to enhance the wellness state or manage problem Resources of the family Resources of the nurse Resources of the community

Gravity or severity of the problem Duration of the problem Current management Exposure of any vulnerable or high-risk group
