Wound Dressing
DEFINITION
It can be defined as total hygiene given to wound,
so as to prevent contamination and promote
healing.
PRINCIPLES INVOLVED IN DRESSING
- Adequate washing of hands before and after
dressing.
- Windows should be close and move of a people
should be restricted, these helps to reduce the
movement of air borne organisms.
- Any Nurse with upper respiratory disease like
common cold ,should be given off duty or should not
allow to dress.
- When applying bandage it should not be too tight
as this will reduce the amount of blood supply to the
area.
- If dressing is soaked with blood it should be re-
pack,if there is no instruction to change from the
doctor.
- Sweeping and dusting should be done an hour
before dressing.
Frequency of changing dressing
Dressing can be changed daily or twice a day
depending upon the doctors order, sometimes
Nurses suggest the frequency with Wich dressing
should be changed and sometimes dressing is
changed whenever it's wet or soaked with blood.
Types of Dressing
1. Wet dressing
2. Dry dressing
3. Open dressing
Function of dressing
. Prevention of infection
. Prevention of further injury
. Control of bleeding
. Reduce pain
. To cover wound in order to prevent
embracement
• Requirements:
A Trolley is required top shelf:
All requirements are sterile.
1. Large tray with lid containing ;
a. Three (3) dressing forceps
b. One pair of dissecting forceps
c. Three gallipots
(i) For lotion
(ii) For gauze
(iii) For cotton wool.
Extra instrument such as stitch removing scissors,pair of scissors,clip
removers to be added P.R.N
2. Large bowl with lid containing : sterile dressing mackintosh and towel.
• Bottom shelf :
All requirement are clean
a. Large tray containing:
(i) Bottle of lotion (ii) bandages and plaster (iii)
A pair of scissors. (iv) If needed antibiotic
powder for spray, vaseline gauze.
b. Large reciever for used instrument containing
savlon 1 in 20 and a pedal bin.
Procedures:
• the dresser assess the patient and explain the procedure to
him/her.
• The dresser and assistant scrub the hands and put on masks.
• Both of them clean the trolley with soap and water or
disinfectant.
• The dresser will set the top shelf using cheatle forceps and
instruct the assistant on setting the bottom shelf.
• The dresser and assistant provide privacy position to the
patient and put off the fan.
• The dresser washes and dries her hands while the assistant
removes the top dressing and washes her hand thereafter.
• The dresser wheels the trolley to the patient bedside while
the assistant carries the pedal bin.
• The assistant open the tray and bowl and pours the lotion
after showing it to the dresser.
• The dresser picks the mackintosh with a pair of
dressing forceps and spread it under the area to
be dressed without contaminating the forceps,
and then places the dressing towel in the same
manner.
• The inner dressing is removed before the
dressing forceps is discarded.
• Using the dressing forceps the dressers proceed
to dress the wound first from the cleanest to the
dirtiest area using each swab once and only in
one direction .
• The wound itself is cleaned in the same manner
using as many swab as needed and discard the
used forceps.
• Prescribe lotion powder or vasline gauze is
applied to the wound.
• Cover the wound with sterile gauze using the
remaining dressing forceps including
dissecting forceps especially in folding the
gauze to remove excess lotion in wet dressing.
• The assistant is intructed to apply bandage
plaster as the case may be .
• The dresser removes all equipment used from
patients beside and the patient is left
comfortable.
