GAWAD 2025-2026 GENERAL GUIDELINES

FLORO CRISOLOGO STUDENT LEADERSHIP AWARD


Recognizing UNP’s highest achieving student leaders. This award is given to graduating student
leaders who have demonstrated excellent overall service through scholarly research presented or
published; awards received extension activities, and campus leadership in the name of the University
and its community during his/her collegiate career.
A. The applicant must:
1. Be a bona fide graduating student leader in a degree program who made exemplary
contributions to a student organization, college, and the community who helped them grow and
achieve institutional goals;
2. Demonstrate a commitment to improve his or her organization's status in the campus;
3. Have at least a General Weighted Average (GWA) of 2.5 with NO FAILING GRADE AND DROPPED
SUBJECT)
4. Have at least two consecutive school years of residency in the University;
5. Have been an officer of at least one (1) mandated or accredited organization in the campus for 2
years as certified by the Office of Student Affairs and Services; and
6. Possess a good moral character issued by the Office of Student Affairs and Services.

B. To complete the process, the student leader must provide the following documents and
submit in hardcopy:
1. A recommendation letter by the Academic Dean for the nominee/s, as endorsed by the
incumbent Organization Adviser. The nominee shall be endorsed by the incumbent
organization adviser for student leaders whose organization serves university-wide;
2. Record of Grades (Certification of Grades) from the Registrar and Appraisal Sheet for the
Senior High School;
3. Only documents with attachments shall be given points; and
4. All documents must be submitted on or before the deadline; otherwise, they shall not be
included in the evaluation.

DEDICACION REYES NON-GRADUATING STUDENT LEADERSHIP AWARD


This award honors non-graduating student leaders who took leadership position/s in an
organization for One (1) School Year (or an up-and-coming leader in other co-curricular areas) but have
already contributed significantly to the quality of campus life at UNP.

A. The applicant must:


1. Be a bona fide undergraduate student leader who made outstanding contributions to a student
organization, college, and the community that have helped them grow and achieve institutional
goals;
2. Have at least a General Weighted Average (GWA) of 2.5 or its equivalent with NO FAILING
GRADE AND DROPPED SUBJECT).
3. Have a residency of at least one school year in the University;
4. Have been an officer of at least one (1) mandated or accredited organization in the campus for 2
years as certified by the Office of Student Affairs and Services; and
5. Possess a good moral character issued by the Office of Student Affairs and Services.

B. To complete the process, the nominee must provide the following documents and submit in
hardcopy:
1. A recommendation letter by the Academic Dean for the nominee/s, as endorsed by the
incumbent Organization Adviser;
2. Record of Grades (Certification of Grades) from the Registrar (until First Term grades of
School Year 2024-2025);
3. Only documents with attachments shall be given points; and
4. All documents must be submitted on or before the deadline; otherwise, they shall not be
included in the evaluation.

VALERIANA ANUNCIACION OUTSTANDING COMMUNITY SERVICE AWARD
This award honors student leaders who have contributed outstanding service to the university
and the community. The service provided by the nominees should be voluntary, non-compensated
activities at the college or in the community. This award is given to students who prioritized service and
impacted the campus and the community through service.

A. The applicant must:


1. Be a bona fide student who made outstanding contributions to a student organization, college,
and the community that have helped them grow and achieve organizational goals;
2. NO FAILING GRADE, INC AND DROPPED SUBJECT during the evaluation period; and
3. Possess a good moral character issued by the Office of Student Affairs and Services.

B. To complete the process, the nominee must provide the following documents and submit in
hardcopy:
1. A recommendation letter by the Academic Dean for the nominee/s, as endorsed by the
incumbent Organization Adviser. The nominee shall be endorsed by the incumbent
organization adviser for student leaders whose organization serves university-wide;
2. Record of Grades (Certification of Grades) from the Registrar and Appraisal Sheet for the
Senior High School;
3. Only documents with attachments shall be given points; and
