    PARENTAL CONSENT

    I/We hereby willingly and voluntarily give consent the participation of my/our
    son/daughter ____________________, __________, currently enrolled at
    RIZAL ELEMENTARY SCHOOL, in the ENHANCEMENT TRAINING IN
    PHOTOJOURNALISM (STREET PHOTOGRAPHY) with SIR BRYAN GUINTO,
    N-October 29-30, 2024, PANABO CITY PARK.

    I have considered the benefits that my son or daughter will derive from his/her
    participation in this activity provided due care and precaution will be observed
    to ensure the health and safety of my son/daughter and that DepEd employees
    and personnel may not be held responsible for any untoward incident that may
    happen beyond their control.

    Further, I/We hereby acknowledge that I/We have been informed of the details
    and purpose of the said activity and I/We hereby authorize the personnel of the
    Department of Education to collect, process, and dispose of the personal
    information of the learner in strict compliance with the provisions of the Data
    Privacy Act of 2012 Republic Act 10173) and the Child Protection Policy of the
    Department.

    I/We give full permission in any recording or picture taken of my child during
    the conduct of this event and to use those in any publication, including
    electronic publications, created by or for the organizers and to release this
    material to official platforms of the school/division.
