___________ will join the Patronal Town Fiesta to be held at Balaoan Civic Center on
September 10, 2023.
I am aware of the usual risks and dangers inherent in the said program thus,
accidents can occur with or without any fault on either the part of the student, or the teachers
where the program is taking place. By allowing my child to participate in this activity, I accept
the risk or any untoward incident, and conform to the school rules and regulations that this
activity is suitable for my son/ daughter in order to enhance his/her skills as a student.
I have read and understood the terms of this consent and waiver, and understood
well that is binding upon me, my heirs, executors and administrators.

Due to unavailability of playing grounds, I am allowing my child to conduct their training at Brgy. Almeida, Balaoan, La
Union Basketball Court. I am also well notified that the practice begins at 3pm on Tuesday, and will be done the whole day on
Wednesday.
I am aware of the usual risks and dangers inherent in the said training thus, accidents can occur with or without any fault
on either the part of the student, or the teachers where the training is taking place. By allowing my child to participate in this activity,
I accept the risk or any untoward incident, and conform to the school rules and regulations that this activity is suitable for my son/
