    CONSENT AND ACKNOWLEDGMENT

    1. I, the undersigned, consent to my child’s participation in the BSP and GSP activity on November 23-
    24, 2024.
    2. I understand that the organizers will take every reasonable precaution for the safety of all
    participants.
    3. In case of any medical emergency, I authorize the organizers and medical staff to administer
    necessary medical treatment to my child.
    4. I understand that the barangay council will assist with peace and order maintenance and that medical
    personnel will be on-site during the event.
    5. I have considered the benefits that my son/daughter will derive from his participation in this activity
    with the understanding that every precaution is to be taken to ensure his safety in camp.
