    LNHS DRUMS & LYRE CORPS SCHEDULE AGREEMENT AND
    LIABILITY WAIVER
    I _____________________________ (name of parent), the undersigned, am fully
    aware and accept the Schedules of the School Band practice, as agreed upon by all
    the parents of the band members who attended the General Assembly held on
    December 1, 2022 in LNHS Computer Laboratory, Brgy. Jubas, Libagon, Southern
    Leyte, as follows:
    1. December 2, 2022 - from 8:00 a.m. to 5:00 p.m.* (Friday)
    2. December 3, 2022 - from 8:00 a.m. to 5:00 p.m.* (Saturday)
    3. December 4, 2022 - from 8:00 a.m. to 5:00 p.m.* (Sunday)
    4. December 5, 2022 - from 8:00 a.m. to 5:00 p.m.* (Monday)
    5. December 6, 2022 - from 8:00 a.m. to 5:00 p.m.* (Tuesday)

    *May be subjected to change upon prior notice

    I fully understand that by signing this agreement, I am allowing my son/daughter


    ________________________________ (name of son/daughter) attend the
    aforementioned schedule of School Band practice as stated above due to limited
    time available for them to master the band execution. Moreover, by signing this
    agreement, I am fully aware that for the following dates, December 2, 2022;
    December 5, 2022, December 6, 2022, my son/daughter will be excused from
    his/her classes with assurance that he/she will be provided with maximum
    consideration and time in complying all the performance tasks and/or other academic
    requirements.
    Hence, by signing this waiver, I recognize that I am giving my full consent to my child
    to attend the band practice, thereby making the Libagon National High School as
    an institution, including all its teaching and non-teaching force, be not subjected to
    any complaints, either administrative or any legal predicaments which may arise due
    to unintentional disruption of classes due to School Band Showdown.

    PARENTAL/GUARDIAN’S CONSENT

    I _____________________________________, the band member’s parent and/or


    legal guardian, allow my son/daughter to undergo on the Agreed Schedules of
    School Band Practice starting DECEMBER 2, 2022 until DECEMBER 6, 2022 at
    the following venues: Barangay Otikon Covered Court, Barangay Tigbao Boulevard,
    and Libagon Rizal Plaza in fulfillment for the approaching SCHOOL BAND
    SHOWDOWN to be held on DECEMBER 8, 2022 in celebration for the Feast Day of
    Immaculate Conception. It is understood that my son/daughter abides by the Safety
    Rules and Regulations that will be imposed by the School Band Coordinators.
    I also agree that while all precautions, including the standard health and safety
    protocols will be taken by the School Band Coordinators, I will not hold the LNHS or
    any of its personnel responsible in case of any untoward incident that may happen
    during the duration of the School Band Practice.
