SBM Accomplishment Report

ACCOMPLISHMENT REPORT

Preliminaries

I. Introduction
In line with the move of the Department of
Education towards the full realization of the DepEd Order
no, 12 s. 2014and DepEd Order no. 13. S. 2016 has the
provisions that schools are provided with financial resources
to implement programs, projects, and activities in their
school improvement plan. Through the School-base
management (SBM) teachers and school heads have the
authority to formulate their own professional development
plans consistent with the organization’s goals and standards.
COVID-19 pandemic brought the educational
system a new setting and made an abrupt change but then,
the Department of Educations is constantly prepared to
whatever plans to ensure the effective delivery of the K-12
Curriculum. Thus, SBM indeed leads to improve school
performance and its policy to expand education by
relocating significant decision-making influence from state
and district offices to individual schools
As John Lindelow and James Heynderickx define
School-based management SBM as a system of
administration in which the school is the primary unit of
educational decision-making." Responsibility for certain
decisions about the budget, personnel, and the curriculum is
placed at the school level rather than the district level,
thereby giving especially principals but also teachers,
students, and parents greater control over the educational
process.   And for the department of education SBM is a
DepEd thrust that decentralizes the decision-making from
the Central Office and field offices to individual schools and
learning centers to enable them to better respond to their
specific educational needs.

Laguna Senior High School (LSHS) had been doing


challenging tasks to sustain an established competent work
process and maintained a competing and advancing school
performance.  Through the years, LSHS has carried out
numerous techniques and strategies which aim to create
quality educational outputs and to develop the learners’
content knowledge and skills in the best possible level.
School Head and the faculty members are enthusiastic to
develop professionally and constantly willing to learn new
skills and develop new insights into pedagogy and their own
practice and explore new or advanced understandings of content
and resources. Laguna Senior High School had
been concentrated their efforts in strengthening support
systems to improve educational planning and management.
The school not only practiced effective delivery of basic
education services but also improved the SBM support
system through interventions of the schools.

II. Highlights of (Priciple 3)


accomplishments  The number and support of stakeholders increased
as evidenced by the records on initiated Programs,
Projects, and Activities of the same.
 Identified gaps addressed:
o Regarding the number of enrollees due to
cessation of offering Technical-Vocational
Livelihood track, this was addressed by
offering Humanities and Social Sciences
(HUMSS) strand. From 617 enrollees when
the strand was first introduced in 2019, it
rose to 1,496 at present.
o The rate of submission of accurate MPS by
teachers increased from 50% to 75% by
capacitating teachers in the use of
technology
 Gulayan sa Paaralan and Feeding Programs were
positively received by the parents based on the
survey conducted by the school.

 Different programs and projects which are product of


innovation are big help to address the gaps in providing
quality services in education. These projects provided
enhanced learning environment that help that school to
reduce the drop-out and failure rate and increase the
number of enrolled students and the achievement rate of
the school.

 The strong partnership of the school and its


stakeholders are highly evident on the implementation
of every project. The stakeholders gave their 100%
support on the programs and project since it prioritized
the improvement not only the school facilities but also
the other educational services that the school can offer.

  Scheduled the proper procedure in identifying what


resources are needed to complete the work, including
the quantity required or the amount of effort required
including any specific skills. Also agree that monitoring
of resources on a daily, weekly, or monthly basis
determines the total amount used at any point in time.

 Good exercise in using resource scheduling to


anticipate and project the resources required to deliver
the work and when they will be required during the
program.

III. Overall Accomplishment of As schools with promised excellence, integrity and passion
Programs and Projects began to soar, an institution with all of those and more has
already risen. In the vast grounds of Laguna’s Provincial
Capital, Laguna Senior High School, formerly known as
Santa Cruz, Capitol Senior High School, kicked off its
pioneer class. LSHS, as what most people call it, had its
official establishment on November 21, 2016. With the aid
of Kautusang Bayan Blg. 22-T2016, or the order that tagged
LSHS as Senior High School No.4 with school number
342346, intensified the very foundation of the school.
            The school is located at Provincial Capital
Compound, Brgy., Poblacion I, Santa Cruz, Laguna. In
LLSHS, three academic tracks are offered namely, Science
Technology Engineering and Mathematics (STEM),
Accountancy, Business and Management (ABM) and
Humanities and Social Sciences (HUMMS). For a very large
stand-alone senior high school, there are 52 teaching
personnels that consists of 1 assistant principal, 8 Master
Teacher II, 2 Master Teacher I, 5 Special Science Teachers
and 36 teachers 1-3 and 4 non-teaching personnel.
            During the establishment of the school in 2016, its
name was Santa Cruz Capitol Senior High School with 86
students and 9 faculty members before it was established as
Laguna Senior High School with almost 2064 students and
52 teaching staff.
            LSHS had numerous events which include but not
limited to: Brigada Eskwela, Sports Fest, Inoubliable Senior
Ball, Founding Anniversary, participation in the District
Meet, Campus Media and Research Conference and
Regional Science and Technology Fair.
            In terms of enrollment, the number of students in the
school year 2017-2018 were 1061, in the school year 2018-
2019 there were 1509 students and recently in the school
year 2019-2020 there were 1475 students who enrolled at
Laguna Senior High School.
            Moreover, for the dropout rate, for school year 2017-
2018 there were 0.18%, 0.003% for school year 2018-2019
and lastly for s.y. 2019-2020, 0.13%.
            The cohort survival rate of Laguna Senior High
School for the school year 2017-2018 were 139%, 98%
during the school year 2018-2019 and 98.75% last school
year 2019-2020.
            The achievement rate of the school is 93.20% during
the school year 2017-2018, 117.02% during the school year
2018-2019 and for the school year 2019-2020, 110.24%.
            Lastly, for the completion rate during the school year
2017-2018, 2018-2019 and 2019-2020 the school attained
100.39%, 98.00 % and 99.12%, respectively.
            Different programs in partnership with stakeholders
were implemented in the school. Projects: Book is
Knowledge (BK), Build-Build-Build-Sports Facilities
(BBB-SF), National Certifications for Academic Strand
(NiCAS), Health Improvement – Leading to Overall Mental
and Physical Wellness (HILOM), MPS for Accurate
Submission Through Exam Reader (MASTER) and Water
Sanitation and Hygiene in School (WASH).
            For Project BK or Book is Knowledge, this project
was headed by Mr. Richard K. Lazaro (Master Teacher II) in
partnership with Hon. Laarni A. Malibiran (Santa Cruz Vice
Mayor). This project aimed to implement system processes,
utilizing books, reference materials, and learning resources
that will be available for students and teachers.
            The next project is the Project BBB-SF or the Build,
Build, Build- School facilities. This project is coordinated
by Mr. Asher H. Pasco (Assistant Principal II), Mrs. Mizpa
Grace R. Cabral (Master Teacher I) and Hon. Benjie Agarao
(Representative District 4). Its target was to develop
partnerships with external stakeholders for the construction
of school facilities.
            For Project NICAS or National Certifications for
Academic Strands, it is designed to increase the number of
NC holders from 3% or 25 out of 918 students of the 2018-
2019. The goal is to increase it by 50% by the end of the
school year 2021-2022. It was spearheaded by Mr. Orlando
A. Peraz (Master Teacher II) and Mr. Justin Jeremiah A.
Ching (Student Representative).
            Project WASH or Water Sanitation and Hygiene in
School was launched with the leadership of Florentino B.
Gutierrez (Master Teacher II) and Pastor Allan Sarmiento
(Parent Representative). The project planned to develop
partnership with external stakeholders for the construction
of the school wash area by the end of the school year 2021-
2022.
            Project HILOM or Health Improvement-Leading to
Overall Mental and Physical Wellness was created to
provide health care assistance to students and to choose a
trained teacher to be a school health coordinator, who will
supervise and manage the school clinic by the end of school
year 2020-2021. Project HILOM was supervised by Ms.
Tessaflor A. Suerte (Master Teacher II) with the
coordination of barangay representative Mr. Ernesto S.
Palomique Jr.

1. Best practices, value-added contributions, innovations, and
other activities
The Laguna Senior High School (LSHS) had been
working hard to maintain an organized and efficient work
process. Through the years, LSHS had carried out various
methods and strategies which aim to create quality
educational outputs and to develop the learners’ content
knowledge and skills in an optimal level. Here are some of
the best practices, value-added contributions, innovations,
and other activities of LSHS which were evident during the
previous years.
a. Utilized a systematic record keeping practices
The Laguna Senior High School (LSHS)
kept an up-to-date record of transactions and an
organized collection of the learners’ and teachers’
documents. This was done with the help of the
School Registrar, the Property Custodian, and the
Administrative Officer. They see to it that every
pertinent document was secured, and every
transaction was properly noted.
b. Ensured stakeholders’ engagement and involvement
The LSHS’ Adopt-a-School Team
spearheaded initiatives which helped ensure that the
stakeholders are highly involved and engaged in the
school activities. Some of the school’s stakeholders
wholeheartedly donated materials and equipment
which were extremely helpful in successfully
delivering instruction to the learners. This made the
attainment of the learning goals more achievable.
c. Held successful fund-raising activities
The school also held fund-raising activities,
with the help of the Parent-Teacher Association,
which became an outlet in acquiring sufficient
budget which aided in the successful
accomplishment of various school projects for the
sake of the learners.
d. Maintained a competitive and progressive school
performance
The Laguna Senior High School (LSHS)
maintained a competitive and progressive school
performance through the years. There is an increase
in enrollment every school year. Some of the
learners and some of the teachers, as well,
participated and won various awards on different
competitions held beyond the school level.
e. Some of the teachers from different disciplines
authored research
Some of the teachers in LSHS authored
research which are helpful in the teaching-learning
process.
f. Implemented successful projects and activities
The school successfully implemented
projects and activities with the collective effort of
the school personnel, learners, and stakeholders.
Such are in the conduct of the “Brigada Eskwela”
and in the preparation for the “Gulayan sa Paaralan”.
Moreover, the school was recognized as the
“Outstanding Brigada Implementer (Secondary/
Large School Category)” and was able to bag the
Third Place in the “Gulayan sa Paaralan (JHS and
SHS Category)” in the Gawad Huwaran 2020.

3a. Narrative on adjustments Made in Basic Education
Learning Continuity Plan (BE-LCP) CONSOLIDATION
TEAM
Pandemic brought a one hundred eighty degree turned to the
educational system not only here in the Philippines but all
over the world. This situation immobilized the flow the
normal routine in every agency and sector of the
government. But the good thing is the Department of
Education did not allow this situation to totally paralyze the
flow of learning delivery here in the Philippines. Instead,
they were able to immediately give alternatives or solution
so to ensure that the teaching and learning process will
continuously happen.

Laguna Senior High School adopted the new normal ways of


delivering knowledge to the students. Adjustment made
were based on the learning continuity plan prepared by
DepEd. Before the school year begins, parents, guardian and
students were contacted by the school to ask them what
mode of learning delivery would be more possible for them
to have. Students were given different learning modalities
such as online learning for those learners who have available
gadgets and stable internet connection at home. Blended
learning for those who have gadgets and able to access the
internet and modular learning wherein the students were
provided printed copies of the self-learning modules. These
options were given by the Department of Education to
ensure that the needs of the learners to have quality
education would still be addressed even though face -to-face
interaction is prohibited. If there are some instructions that
are hard for the students to answer, the teachers made
themselves available to answer all the queries of the students
through phone calls or through social media.

Those parents who are required to go to school for the card


signature or for the distribution of SLMs were scheduled so
that coming of people in school would be limited.

The security and safety of both teaching and non-teaching


personnel were also prioritized that’s why works of the
teachers were done at home. Meetings, conferences and
submission of reports and pertinent papers needed were
done online. If there was a need arise wherein the teacher is
needed to visit the school for some important reason, they
were instructed to strictly follow the health protocols.
Teachers before worked collaboratively while enjoying a
face-to-face interaction but now they must work distancing
themselves to one another to ensure not only their safety but
also of their loved ones.

These were some of the adjustments done so to ensure that


learning would still continuous despite the public health
situation that we are experiencing.

3b. Narrative on coping strategies/change of work processes


in accordance with health and safety protocols ALL

The development and implementation of Learning
Continuity Plan for education to continue in times of
pandemic is also anchored and in reference to existing
community and national health protocols and safety
measures. Laguna Senior High School has adopted
numerous coping strategies and work processes to deliver
service to its valued clienteles and stakeholders.

Work-from-Home Arrangement was employed where


teaching and non-teaching personnel perform their work
duties at their homes. Physical reporting, when necessary,
was also in accordance with safety protocols. In a limited
number, teaching, and non-teaching personnel report on
school site to accomplish tasks relative to the
implementation of the Learning Continuity Plan. Most of the
times, teachers report on school to prepare the Self-Learning
Modules and other instructional materials needed in a
specified learning period.

At times teaching and non-teaching personnel report at


school, and whenever clients and visitors are present,
physical distancing was observed. The wearing of face
masks and face shields was also practiced. Before anyone
can the enter the vicinity, the temperature is checked, and
health declaration and contract-tracing documents are filled-
in. Designated wash and disinfecting areas are also available
so that visitors are required to wash their hands and disinfect
prior to proceeding to the offices. Marks and seats are also
provided so that physical distancing is strictly ensured.

The distribution of self-learning modules and retrieval of


outputs and answer sheets are all conducted in a scheduled
basis. Class advisers would assign specific time for small
groups for the module distribution. The office and the class
advisers would coordinate with students, parents, and
stakeholders regarding the schedule of printed module
distribution and output submission to ensure that number of
persons in the vicinity would only be at a minimum.
Teachers and school personnel transact with parents in a
physically separated working area to limit contact and
exposure.

Meetings, conferences, and training sessions were all


conducted virtually. The submission, review, and checking
of pertinent work documents were also done online. To
ensure that all school personnel would have a very little
chance of contracting the virus on site or on the way to
school site, submission of printed reports are done only
when required.

These implemented work processes and strategies ensured


that safety protocols were observed while service is
delivered to learners and stakeholders.

VI. Financial Performance PRINCIPLE 3


For 2019, LSHS Maintenance and other Operating Expenses
amounted to P2,026,000. The bulk of which was used in
purchasing supplies for programs and projects identified and
approved in the ESIP; and materials for administrative. Said
fund was also utilized to pay the expenses for utilities and
for manpower and services as well.

Meanwhile, a total of P195,294 was also sourced from the


operation of the school’s canteen. The said earnings were
utilized for the following: School Feeding Fund, School
Clinic Fund, Faculty and Student Development Fund, HE
Instructional Fund, School’s Operation Fund, and Revolving
Capital of the canteen.

The School’s GPTA conducted a fund-raising activity and


reportedly collected P100,000 in total. This was utilized in
the construction of LSHS’s ornate signage.
During the year, the Adopt-A-School Program garnered
P70,000 from our stakeholders which was utilized in the
construction of the school’s gate for the safety and security
of the learners, school personnel, and the premises in
general.
