1. PURPOSE :
1.1 To ensure all patients are assessed appropriately by nurses to meet the needs of individual patients. 1.2 To establish the baseline information about the patient. 1.3 To ensure each patient receives the highest standard of care.

2. DEFINITION:
2.1 It is the systematic and continuous collection, validation and communication of patients data.

3. POLICY: 3.1 Gathering of information about patient, begin when the patient approaches the health care
system in the first time. 3.2 The Nursing Staff shall perform a complete assessment and re - assessment of the patient which include physical,psychological and social aspects.

4. PROCEDURES :
4.1 Select an appropriate place to perform assessment. 4.2 Inform the patient of their mutual roles and responsibilities in the assessment and encourage the patients participation. 4.3 Utilize interview, behavioural observation, physical and mental status and assessment skill to achieve accurate assessment of patient care needs. 4.4 Re- assess the patient throughout the course of treatment. 4.5 Inform Physician when a significant change occurs.
