The Cayuga County Clerk's Office has implemented the Affidavit Concerning Survey Map (ACSM) form to be used when you are filing all stand alone survey maps in our office.

Our Affidavit Concerning Survey Map form is to be used when you file a stand-alone survey map in our office. There is no charge for the Affidavit Concerning Survey Map form, however, there is a $10 fee for filing a map. If presented at time of filing, additional identical original maps can be file stamped at no charge.
Mailing Maps
If you are mailing your maps, be sure to place your Affidavit Concerning Survey Map form on top, along with your fees.
Map Filing Requirements
- 2 Paper maps with signature & seal
- Appropriate fees
- Appropriate surveyor, town / village and health department stamps / signatures
- Completed and signed affidavit concerning survey map form
