A standard form contract (sometimes referred to as an adhesion or boilerplate contract) is a contract between two parties, where the terms and conditions of the contract are set by one of the parties, and the other party has little or no ability to negotiate more favorable terms and is thus placed in a "take it or leave it" position.
Examples of standard form contracts are insurance policies (where the insurer decides what it will and will not insure, and the language of the contract) and contracts with government agencies (where certain clauses must be included by law or regulation).
While these types of contracts are not illegal per se, there exists a very real possibility for unconscionability. In addition, in the event of an ambiguity, such ambiguity will be resolved contra proferentem against the party drafting the contract language.
Theoretical issues
There is much debate on a theoretical level whether, and to what extent, courts should enforce standard form contracts.
Standard Form 50 (or SF-50), is a Notification of Personnel Action, maintained by the National Personnel Records Center of the National Archives and Records Administration
standard form is used for many things. Its main use is to find lateral equations.
Standard Form 180 (SF-180, the Request Pertaining to Military Records) is a one-page authorization form (plus 2 pages of instructions) of the U.S. military. The form may be filled out by veterans of the U.S. military or their surviving next-of-kin to view and/or release a person's military record.
The SF 180 may also be used to request a one-time replacement of Medals earned in service.
You can obtain your military records form the National Personnel Records Center or the military if you were discharged recently. You may also find private researchers who will obtain your record for a fee. The National Archives has a list of independent researchers who will undertake a personal search for you.
Standard Form 312 (SF 312) is a non-disclosure agreement required under Executive Order 13292 to be signed by employees of the U.S. Federal Government or one of its contractors when they are granted a security clearance for access to classified information. The form is issued by the Information Security Oversight Office of the National Archives and Records Administration and its title is "Classified Information Nondisclosure Agreement." SF 312 prohibits confirming or repeating classified information to unauthorized individuals, even if that information is already leaked. The SF 312 replaces the earlier forms SF 189 or the SF 189-A. Enforcement of SF-312 is limited to civil actions to enjoin disclosure or seek monetary damages and administrative sanctions, "including reprimand, suspension, demotion or removal, in addition to the likely loss of the security clearance."

A standard form contract (sometimes referred to as an adhesion or boilerplate contract) is a contract between two parties, where the terms and conditions of the contract are set by one of the parties, and the other party has little or no ability to negotiate more favorable terms and is thus placed in a "take it or leave it" position.
Examples of standard form contracts are insurance policies (where the insurer decides what it will and will not insure, and the language of the contract) and contracts with government agencies (where certain clauses must be included by law or regulation).
While these types of contracts are not illegal per se, there exists a very real possibility for unconscionability. In addition, in the event of an ambiguity, such ambiguity will be resolved contra proferentem against the party drafting the contract language.
Theoretical issues
There is much debate on a theoretical level whether, and to what extent, courts should enforce standard form contracts.
