The USMLE Step 1 (more commonly just Step 1 or colloquially, The Boards) is the first part of the United States Medical Licensing Examination. It assesses whether medical school students or graduates can apply important concepts of the sciences fundamental to the practice of medicine. US medical students, as well as Canadian medical students who wish to seek licensure in the US, typically take Step 1 at the end of the second year of medical school. Graduates of international medical schools (i.e., those outside the US or Canada) must also take Step 1 if they want to practice in the US. Graduates from international medical schools must apply through ECFMG, and the registration fee is $850. For 2015, the NBME registration fee for the test is $590, with additional charges for applicants who choose a testing region outside the United States or Canada.
Format
The exam is an eight-hour computer-based test taken in a single-day, composed of seven 44-question sections with a total of 308 multiple-choice questions. One hour is provided for each section, allotting an average of a minute and eighteen seconds to answer each question. Between test sections, the test taker is allotted a cumulative 45 minutes (during the test day) for personal breaks. (There is a 15-minute tutorial at the beginning of the exam, which the test-taker can choose to skip and have that time added to break time.) If the taker finishes any section before the allotted one hour time limit, the unused time is added to the break time total. The test is administered at any of several Prometric computer testing sites.
Onchocerca volvulus is a nematode that causes onchocerciasis or "river blindness" mostly in Africa. Long-term corneal inflammation, or keratitis, leads to thickening of the corneal stroma which ultimately leads to blindness. Humans are the only definitive host for O. volvulus. The intermediate host or vector is the black fly (Simulium).
O. volvulus, along with most filarial nematodes, share an endosymbiotic relationship with the bacterium Wolbachia. In the absence of Wolbachia, larval development of the O. volvulus is disrupted or ceased.
The life cycle of O. volvulus begins when a parasitised female black fly of the genus Simulium takes a blood meal. The microfilariae form of the parasite found in the dermis of the host is ingested by the black fly. Here the microfilariae then penetrates the gut and migrates to thoracic flight muscles of the black fly, entering its first larval phase (J1). After maturing into J2, the second larval phase, it migrates to the proboscis where it can be found in the saliva. Saliva containing stage three (J3S) O. volvulus larvae passes into the blood of the host. From here the larvae migrate to the subcutaneous tissue where they form nodules and then mature into adult worms over a period of six to twelve months. After maturation, the smaller adult males migrate from nodules to subcutaneous tissue where they mate with the larger adult females, which then produce between 1,000 and 3,000 microfilariae per day. The normal adult worm lifespan is up to fifteen years. The eggs mature internally to form stage one microfilariae, which are released from the female's body one at a time and remain in the subcutaneous tissue.

How USMLE Step 1 Pass/Fail is Changing Medical School
The United States Medical Licensing Examination, or USMLE, is a 3-part test designed to assess one’s medical knowledge and application of said knowledge to patient care. Step 1 is typically taken at the end of the second year of medical school and is followed by Step 2 CK at the end of the third year or sometime during the fourth year.
Up until now, Step 1 has been arguably the most important test a future physician will ever take. The Fly Catchers fighting river blindness
Onchocerciasis, commonly known as river blindness, is a parasitic disease transmitted by the bites of the black flies, the flies breed in fast-flowing rivers and streams.
As the parasites reproduce, hundreds of thousands of larvae spread throughout the person's body. These grow into adult worms, up to 30cm long Eventually it leads to blindness.
Charles and John work as fly catchers in Uganda. Their job involves catching the flies before getting bitten, they distribute medicine to the community and spray the river with eco-friendly chemicals.

Onchocerciasis is an eye and skin disease caused by a worm (filaria) known scientifically as Onchocerca volvulus. It is transmitted to humans through the bite of a blackfly (simulium species). These flies breed in fast-flowing streams and rivers, increasing the risk of blindness to individuals living nearby, hence the commonly known name of "river blindness". Within the human body, the adult female worm (macrofilaria) produces thousands of baby or larval worms (microfilariae) which migrate in the skin and the eye.

The United States Medical Licensing Examination, or USMLE, is a 3-part test designed to assess one’s medical knowledge and application of said knowledge to patient care. Step 1 is typically taken at the end of the second year of medical school and is followed by Step 2 CK at the end of the third year or sometime during the fourth year.
Up until now, Step 1 has been arguably the most important test a future physician will ever take. If you wanted to match into a highly competitive specialty like dermatology or plastic surgery, for instance, but you didn’t score high enough on Step 1, you could pretty much kiss your dream specialty goodbye. Many medical students have had to completely change their career trajectory due to a low Step 1 score and pursue entirely different specialties than they had initially intended.
As of January 2022, however, this will no longer be the case. USMLE Step 1 will be graded as pass/fail and will no longer be the primary determinant of one’s competitiveness as a residency applicant. So how is this change actually impacting the medical school experience? Let's find out.

The USMLE Step 1 (more commonly just Step 1 or colloquially, The Boards) is the first part of the United States Medical Licensing Examination. It assesses whether medical school students or graduates can apply important concepts of the sciences fundamental to the practice of medicine. US medical students, as well as Canadian medical students who wish to seek licensure in the US, typically take Step 1 at the end of the second year of medical school. Graduates of international medical schools (i.e., those outside the US or Canada) must also take Step 1 if they want to practice in the US. Graduates from international medical schools must apply through ECFMG, and the registration fee is $850. For 2015, the NBME registration fee for the test is $590, with additional charges for applicants who choose a testing region outside the United States or Canada.
Format
The exam is an eight-hour computer-based test taken in a single-day, composed of seven 44-question sections with a total of 308 multiple-choice questions. One hour is provided for each section, allotting an average of a minute and eighteen seconds to answer each question. Between test sections, the test taker is allotted a cumulative 45 minutes (during the test day) for personal breaks. (There is a 15-minute tutorial at the beginning of the exam, which the test-taker can choose to skip and have that time added to break time.) If the taker finishes any section before the allotted one hour time limit, the unused time is added to the break time total. The test is administered at any of several Prometric computer testing sites.

Onchocerciasis, commonly known as river blindness, is a parasitic disease transmitted by the bites of the black flies, the flies breed in fast-flowing rivers and streams.
As the parasites reproduce, hundreds of thousands of larvae spread throughout the person's body. These grow into adult worms, up to 30cm long Eventually it leads to blindness.
Charles and John work as fly catchers in Uganda. Their job involves catching the flies before getting bitten, they distribute medicine to the community and spray the river with eco-friendly chemicals.

Onchocerciasis is an eye and skin disease caused by a worm (filaria) known scientifically as Onchocerca volvulus. It is transmitted to humans through the bite of a blackfly (simulium species). These flies breed in fast-flowing streams and rivers, increasing the risk of blindness to individuals living nearby, hence the commonly known name of "river blindness". Within the human body, the adult female worm (macrofilaria) produces thousands of baby or larval worms (microfilariae) which migrate in the skin and the eye.
