On the afternoon of May 15, 2017, a Learjet 35A on a positioning flight from Philadelphia to Teterboro, NJ, crashed while circling to land at Teterboro in visual conditions.
The NTSB released disturbing audio transcripts from the cockpit voice recorder in the Learjet. The transcripts reveal dysfunctional cockpit operations between a task-saturated captain and an inexperienced first officer, flying a demanding airplane in one of the most intense airspace regions in the world.
The AOPA Air Safety Institute examines the links in the accident chain, and discusses how to apply the lessons learned from a commercial flight gone wrong.

Accident Case Study: Faulty Assumptions
In the early morning light on April 22, 2019, the pilot of a Beechcraft Baron 58 preflights his aircraft at West Houston Airport in Texas, before he heads to the terminal to greet his five passengers.
The flight will take just over one hour. But the pilot’s mistaken beliefs about existing onboard fuel will prove disastrous and clinch the flight’s outcome.

There are several different types of emergency landings for powered aircraft: planned landing or unplanned landing
Forced landing – the aircraft is forced to make a landing due to technical problems. Landing as soon as possible is a priority, no matter where, since a major system failure has occurred or is imminent. It is caused by the failure of or damage to vital systems such as engines, hydraulics, or landing gear, and so a landing must be attempted where a runway is needed but none is available. The pilot is essentially trying to get the aircraft on the ground in a way which minimizes the possibility of injury or death to the people aboard. This means that the forced landing may even occur when the aircraft is still flyable, in order to prevent a crash or ditching situation.
Precautionary landing may result from a planned landing at a location about which information is limited, from unanticipated changes during the flight, or from abnormal or even emergency situations. This may be as a result of problems with the aircraft, or a medical or police emergency. The sooner a pilot locates and inspects a potential landing site, the less the chance of additional limitations being imposed by worsening aircraft conditions, deteriorating weather, or other factors.
Ditching is the same as a forced landing, only on water. After the disabled aircraft makes contact with the surface of the water, the aircraft will most likely sink if it is not designed to float, although it may float for hours, depending on damage.
Debris left on Runway 22, Rostov-on-don Airport of Flydubai Flight 981
January 8 - West Air Sweden Flight 294, a Bombardier CRJ200 cargo freighter, crashes while in cruise near Akkajaure in Sweden. Both crew members on board are killed.
February 2 - Daallo Airlines Flight 159, an Airbus A321, suffers an explosion shortly after taking off from Aden Adde International Airport, Somalia. Two people are injured and one, the suspected suicide bomber, is killed after falling from the aircraft.
February 24 - Tara Air Flight 193, a Viking Air-built DHC-6 Twin Otter, flies into a storm and crashes into a mountain side at Dana, Myagdi district, Nepal killing all 23 on board.
February 26 - 2016 Air Kasthamandap crash, an Air Kasthamandap PAC 750XL crash lands, killing the two crew members and injuring nine passengers in Nepal.
March 19 - Flydubai Flight 981, a Boeing 737-800, crashes while landing at Rostov-on-Don, Russia, in poor weather. All 62 people on board are killed.
March 29 - EgyptAir Flight 181, an Airbus A320, is hijacked and forcefully diverted to Larnaca International Airport, Cyprus. All passengers and crew are released unharmed.
April 4 - Batik Air Flight 7703, a Boeing 737-800, collides with an ATR 42 on the runway at Halim Perdanakusma Airport in Jakarta. Both aircraft are substantially damaged.
April 13 - In the 2016 Sunbird Aviation crash, a Britten-Norman Islander crashes short of the runway while landing at Kiunga, Papua New Guinea. All 12 people on board are killed.

Precautionary landing may result from a planned landing at a location about which information is limited, from unanticipated changes during the flight, or from abnormal or even emergency situations. This may be as a result of problems with the aircraft, or a medical or police emergency. The sooner a pilot locates and inspects a potential landing site, the less the chance of additional limitations being imposed by worsening aircraft conditions, deteriorating weather, or other factors.
Ditching is the same as a forced landing, only on water. After the disabled aircraft makes contact with the surface of the water, the aircraft will most likely sink if it is not designed to float, although it may float for hours, depending on damage.

January 8 - West Air Sweden Flight 294, a Bombardier CRJ200 cargo freighter, crashes while in cruise near Akkajaure in Sweden. Both crew members on board are killed.
February 2 - Daallo Airlines Flight 159, an Airbus A321, suffers an explosion shortly after taking off from Aden Adde International Airport, Somalia. Two people are injured and one, the suspected suicide bomber, is killed after falling from the aircraft.
February 24 - Tara Air Flight 193, a Viking Air-built DHC-6 Twin Otter, flies into a storm and crashes into a mountain side at Dana, Myagdi district, Nepal killing all 23 on board.
February 26 - 2016 Air Kasthamandap crash, an Air Kasthamandap PAC 750XL crash lands, killing the two crew members and injuring nine passengers in Nepal.
March 19 - Flydubai Flight 981, a Boeing 737-800, crashes while landing at Rostov-on-Don, Russia, in poor weather. All 62 people on board are killed.
March 29 - EgyptAir Flight 181, an Airbus A320, is hijacked and forcefully diverted to Larnaca International Airport, Cyprus. All passengers and crew are released unharmed.
April 4 - Batik Air Flight 7703, a Boeing 737-800, collides with an ATR 42 on the runway at Halim Perdanakusma Airport in Jakarta. Both aircraft are substantially damaged.
April 13 - In the 2016 Sunbird Aviation crash, a Britten-Norman Islander crashes short of the runway while landing at Kiunga, Papua New Guinea. All 12 people on board are killed.
