Prevention of male pattern hair loss or androgenic alopecia in 20’s is premature obviously. But there is nothing much that can be done to prevent this. Once the genes are passed onto you, then you will. Early presentation of genes will lead to early baldness, there are medications to be given. You can consult your dermatologist, there are lotions to be given to grow your hair and these tablets can be given under the prescription only. You cannot take it over the counter. You can be helped. Atleast you can save whatever hairs you already have. Also prevent further shedding of the hair and as much as possible, we can help you to grow your hair back also. But t is a continuous long term treated. The minute you stop, you can loose back your hair.

After analysing the scalps of bald men, the scientists found that the levels of the PGD2 protein were three times higher in the balding areas of the head.

I am Dr Chen Yiming.
I’ll be talking about Male pattern hair loss ( androgenetic alopecia) . In a normal healthy head of hair, the follicles on our scalp can be in three phases.
1) The anagen phase is known as the growth phase.
About 80% of our hair should be in this phase and it lasts a good 3-5 years. The hair is receiving good nourishment from our blood circulation and is constantly growing.
2) The catagen phase - otherwise known as the transition phase.
Only a small amount of hair are in this phase. This is a short 2 weeks transition stage where the hair is detaching from the follicle and the blood supply and the growth of the hair stops.
3) The Telogen phase - about 20% of our hair is at this stage and it can last for about 2-3 months. This is the phase where the old hair drops off and new hair starts to grow.
The male hormone that is responsible for causing hair loss is Testosterone. Testosterone is converted into dihydrotestosterone by an enzyme called 5a-reductase within our body.
This Dihydrotestosterone ( DHT for short ) will bind to the hair follicles and cause
1) accelerated conversion from anagen phase to telogen phase - meaning more hair fall and lesser hair growth
2) Over time, it will also cause the hair follicles to shrink in size and as a result, the hair becomes thinner and finer
3) Finally, the hair follicle will close up and stop producing anymore hair.
The longer this process is allowed to continue - the more irreversible the condition becomes.
So treatment has to start as soon as you notice increased hair thinning.
So who are at risk of Male pattern hair loss
1) Family history : If we have a close relative with male pattern hair loss, for example our uncle or father, then our risk of suffering from it is higher
2) Age plays a part: the older we get, the higher the chance.
One that is commonly used is the Norwood Hamilton scale. We use this scale to help identify which stage our patient is at and it also shows the severity of the condition.
In stage 1, there is mild recession of the frontal hairline
From stage 4 onwards, there is also balding seen at the top of the head, an area we call the vertex.
At stage 6 and beyond, the frontal recession will meet with vertex hair loss. Stage 7 is the worst where the patient will lose all his hair at the crown.
What are the treatment options to reverse this?
I would subdivide them into
1) Topical application
2) Oral medications
3) Treatments
The only proven topical application that helps will be minoxidil spray.
The exact mechanism of Minoxidil is not determined yet but animal studies found that it could potentially shorten the dying phase of hair follicles and prolong the growth phase. It has also been shown to increase the size of follicles. It can potentially improve hair growth in men over 6 months following which they have to use it for the long run. It can occasionally cause temporary hair loss for the first 2-3 months and sometimes scalp irritation and itch.
The Oral medication that we often prescribe is finasteride - the brand name is propecia. Propecia is only licensed for use in men.
What it does is that it blocks the conversion of testosterone into DHT and in doing so, prevents the premature thinning of hair and premature closure of follicles. It is a very very effective treatment but there are some side effects to taking this medication, so please have a detail consultation with your doctor.
Some of the treatments that may be used in conjunction with minoxidil and propecia are
1) Lasers
Firstly , low level laser therapy has been extensively studied. This low energy red or almost infra red light can be applied on the scalp daily and that has been proven to be a very safe method in stimulating hair growth.
In recent studies, there are also exploration into using semi ablative fractional lasers in the treatment of male pattern hair loss.
2) Hair transplantation.
The most invasive treatment of all would be hair transplants.
When all else fails, patients may wish to consider this surgical approach. Newer methods of treatment that are being evaluated including PRP and Engineered growth factor injections.

That means you are giving time to prevent the hair thinning at a faster rate. There are certain treatments which the person needs to know.one is the hair loss brushes. One persistent hair loss myth is stimulating the scalp with magnets, brushes and massages can improve blood circulation to the hair follicles and therefore recue hair loss and improve new hair growth. There is no reliable medical evidence to support this claim, while there are real medical treatments to stimulate hair follicles and help improve blood circulation like minoxidil, though there was a laser therapy, use of platelet rich plasma, or platelet rich fibrin. Next herbal supplements. Use of supplements like biotin and marine derived proteins and polysaccharide help support hair quality. However it is important to keep expectations realistic. A vitamin is not going to stop hereditary hair loss or regrow hair follicles from scalp where hair is already dead and gone. Only FDA approved medical treatment like minoxidil and finasteride have been proven to only slow the hereditary hair loss and surgical transplantation can regrow hair where severe depletion of hair follicles has occurred. Hair transplant is a promising solution to baldness. We need to know that hair transplants is not an initial solution to hair thinning or initial stages of hair loss. This involves moving the hair follicles from the donor area to the less hair bearing areas of the scalp. Few things which needs to be considered are the age of the patient, the degree of baldness, the stability and the availability of the donor for hair transplantation. the good news is that there are many hair loss treatments that can work well for men and women when used correctly and consistently in proper combinations depending on the hair loss status and goals like follicular unit extraction hair transplantation, specially formulated minoxidil, low level laser therapy and scientifically proven supplements. But you need to know that the hair loss treatments are only to salvage the hair till its course and effect of its regular application are not permanent. Only a hair transplant with meticulous guidelines gives a permanent result.
