Cationic Antimicrobial Proteins (CAPs)
The secondary granules or specific granules in neutrophils contain CAPs including defensins, lysozymes, and Cathelicidins.
The CAPs are characterized by their positive net charge
Cathelicidins are a family of antimicrobial peptides that are stored in neutrophil granules as inactive precursors called pro-cathelicidins.
Upon activation by proteolytic cleavage (removal of pro-peptide region), cathelicidins release active peptides that exhibit antimicrobial activity by disrupting microbial membranes and also modulating immune responses.
Cathelicidins exhibit broad-spectrum antimicrobial activity against bacteria, fungi, and some enveloped viruses
In humans, the most extensively studied cathelicidin is LL-37 (+6), derived from the precursor protein hCAP18 (human cationic antimicrobial protein 18)
Mechanism of Action
Cathelicidins like LL-37 possess an amphipathic structure (have both hydrophobic and hydrophilic regions) and this amphipathic nature allows them to interact with lipid bilayers of microbial membranes.
Upon encountering a microbial membrane, LL-37 is inserted into the lipid bilayer, with its hydrophobic regions buried within the lipid acyl chains and their cationic regions oriented towards the membrane surface.
The interaction of LL-37 with microbial membranes leads to the formation of pores and compromises the integrity and permeability of the membrane.
This causes leakage of intracellular contents and ultimately leads to microbial cell death.
The pore-forming activity of cathelicidins is often associated with their ability to oligomerize or aggregate on the membrane surface, leading to the formation of transmembrane pores or defects.
LL-37 can also interact with intracellular targets in bacteria, such as DNA and proteins, disrupting essential cellular processes.
How Selective Targeting is Achieved
Cathelicidins exhibit selective targeting of microbial membranes over mammalian cell membranes.
This selectivity is attributed to differences in membrane composition between microbial and mammalian cells, such as differences in lipid composition and surface charge.
Microbial membranes typically contain a higher proportion of negatively charged lipids, such as phosphatidylglycerol and cardiolipin, which provide electrostatic interactions with the positively charged cationic regions of cathelicidins, facilitating their binding and membrane disruption.

Cathelicidins are an ancient class of antimicrobial peptides (AMPs) with broad spectrum bactericidal activities. In this study, we investigated the diversity and biological activity of cathelicidins of buffalo, a species known for its disease resistance. A series of new homologs of cathelicidin4 (CATHL4), which were structurally diverse in their antimicrobial domain, was identified in buffalo. AMPs of newly identified buffalo CATHL4s (buCATHL4s) displayed potent antimicrobial activity against selected Gram positive (G+) and Gram negative (G-) bacteria. These peptides were prompt to disrupt the membrane integrity of bacteria and induced specific changes such as blebing, budding, and pore like structure formation on bacterial membrane. The peptides assumed different secondary structure conformations in aqueous and membrane-mimicking environments. Simulation studies suggested that the amphipathic design of buCATHL4 was crucial for water permeation following membrane disruption. A great diversity, broad-spectrum antimicrobial action, and ability to induce an inflammatory response indicated the pleiotropic role of cathelicidins in innate immunity of buffalo. This study suggests short buffalo cathelicidin peptides with potent bactericidal properties and low cytotoxicity have potential translational applications for the development of novel antibiotics and antimicrobial peptidomimetics.

Simulation studies suggested that the amphipathic design of buCATHL4 was crucial for water permeation following membrane disruption. A great diversity, broad-spectrum antimicrobial action, and ability to induce an inflammatory response indicated the pleiotropic role of cathelicidins in innate immunity of buffalo. This study suggests short buffalo cathelicidin peptides with potent bactericidal properties and low cytotoxicity have potential translational applications for the development of novel antibiotics and antimicrobial peptidomimetics.
