Interventional radiology (IR), also known as vascular and interventional radiology (VIR) or surgical radiology, is an independent medical specialty (previously a sub-specialty of radiology) providing minimally invasive image-guided diagnosis and treatment of diseases in every organ system. Although the range of procedures performed by interventional radiologists is broad, the unifying concept behind these therapies is the use of the most modern, least invasive technique available in order to minimize risk to the patient and improve health outcomes.
As the inventors of angioplasty and the catheter-delivered stent, interventional radiologists pioneered modern minimally invasive medicine. Using X-rays, CT, ultrasound, MRI, and other imaging modalities, interventional radiologists obtain images which are then used to direct interventional instruments throughout the body. These procedures are usually performed using needles and narrow tubes called catheters, rather than by making large incisions into the body as in traditional surgery.

Dr Clare Bent, Interventional Radiology Consultant, performs a varicocele embolization on a male patient at The Royal Bournemouth Hospital. This pin hole surgery through the jugular vein will help improve his fertility. The procedure was live streamed to a meeting at the hospital of local MP Desmond Swayne and several local councillors to highlight the benefits interventional radiology can give to patients as a minimally invasive alternative to open surgery. The Royal Bournemouth Hospital is the major Interventional Radiology centre for Dorset and south Wiltshire, providing specialist services for a population of over 1,000,000, including emergency repair of complex aneurysms.

Vessel puncture is performed at free-hand or under ultrasound/fluoroscopic guidance with a 18G cannula. Once the needle is in the vascular lumen a guidewire is passed through the needle and an arterial vascular sheath is finally placed in the artery. On the basis of the CT examination a diagnostic catheter is placed in large vessel as near as possible to the suspected bleeding vessels. A diagnostic angiography run is performed and bleeding site is usually identified. A microcatheter is moved to the site of bleeding and embolization is performed with the most appropriate embolic agent (Coils, Glue, PVA, Gelfoam, Plug, Stent, ecc…). A final diagnostic angiographic run confirms the bleeding cessation.

What is an Interventional Radiologist?
Interventional radiology is a medical sub-specialty of radiology utilizing minimally-invasive image-guided procedures to diagnose and treat diseases in nearly every organ system. The concept behind interventional radiology is to diagnose and treat patients using the least invasive techniques currently available in order to minimize risk to the patient and improve health outcomes. These procedures have less risk, less pain and less recovery time in comparison to open surgery.
Interventional radiologists are medical doctors with additional six or seven years of specialized training after medical school. All faculty interventionalists have completed a one or two-year fellowship program after their diagnostic radiology residency. They are certified by the American Board of Radiology.

Trauma is still a challenge for many professional figures in the medical field. When the patient is bleeding a rapid and effective management is required to prevent cardiovascular shock and death.
Interventional radiology plays a key role in the stable patient, providing multiple treatment options .
DIAGNOSTIC WORKUP
Angiographic-CT is the imaging test of choice in patients with suspected bleeding. Basal acquisition is performed for the identification of the hematoma and bone fractures. Arterial, Portal and Venous phases are subsequently performed in order to identify the bleeding vessel.
PROCEDURAL STEPS AND TECHNIQUE
Vascular access is performed usually at the common femoral or radial artery site. Pain control is achieved with local anesthesia. Vessel puncture is performed at free-hand or under ultrasound/fluoroscopic guidance with a 18G cannula. Once the needle is in the vascular lumen a guidewire is passed through the needle and an arterial vascular sheath is finally placed in the artery. On the basis of the CT examination a diagnostic catheter is placed in large vessel as near as possible to the suspected bleeding vessels. A diagnostic angiography run is performed and bleeding site is usually identified. A microcatheter is moved to the site of bleeding and embolization is performed with the most appropriate embolic agent (Coils, Glue, PVA, Gelfoam, Plug, Stent, ecc…). A final diagnostic angiographic run confirms the bleeding cessation.

Interventional radiology (IR), also known as vascular and interventional radiology (VIR) or surgical radiology, is an independent medical specialty (previously a sub-specialty of radiology) providing minimally invasive image-guided diagnosis and treatment of diseases in every organ system. Although the range of procedures performed by interventional radiologists is broad, the unifying concept behind these therapies is the use of the most modern, least invasive technique available in order to minimize risk to the patient and improve health outcomes.
As the inventors of angioplasty and the catheter-delivered stent, interventional radiologists pioneered modern minimally invasive medicine. Using X-rays, CT, ultrasound, MRI, and other imaging modalities, interventional radiologists obtain images which are then used to direct interventional instruments throughout the body. These procedures are usually performed using needles and narrow tubes called catheters, rather than by making large incisions into the body as in traditional surgery.
