De-Grading Education | Elizabeth Wissner-Gross | TEDxBeaconStreet
Elizabeth Wissner-Gross, author of best-selling books on education, and Founder and President of Educational Strategies, shares her epiphany on why American schools lag far behind those of other countries. Hint: It's not the teachers' fault. It's not the students, parents, unions, tenure, too much homework, too little homework, or even too much testing. The problem, pure and simple, is grades and fear of grade inflation. The American grading system forces our teachers to "Anti-Teach." This TEDxBeaconStreet talk, excerpted from the author's upcoming book "Gotcha Grading," reveals how eliminating our grading system could bring American education back up to the top.

...that's all a little dramatic, but that really is the goal of this video. Our schools are broken, our workplaces are broken, and our society is broken...and it's all because of bad psychology and bad economics.

Why the traditional grading system does more harm than good | Chip Porter | TEDxYouth@MBJH
Having delivered humorous TEDx talks before, Chip decided to do something very different for this talk. Imaging brain tumors - 3 - Oligodendrogliomas
Brain tumors are one of the most common diagnoses addressed in neuroradiology. This covers a wide spectrum of disease, from primary brain tumors like gliomas and glioblastomas to secondary disease like metastases. This lecture covers the spectrum of the most common brain tumors, with an emphasis on primary brain tumors.
This video discusses imaging and genetic features of oligodendrogliomas, including low grade oligodendrogliomas (WHO grade 2) and anaplastic oligodendroglioma (grade 3). By definition, these tumors have 1p19q codeletion and IDH mutation. Oligodendrogliomas have better survival than astrocytomas. Higher grade tumors are more likely to have more mass effect, more enhancement, and less well-defined margins. However, oligodendrogliomas typically do not degenerate into glioblas...

Tumor grade and stage are two critical factors used to describe and classify cancerous tumors. They provide important information about the nature and extent of cancer, which is crucial for treatment planning and predicting the prognosis of a patient. Let's delve into each of these concepts:
1. Tumor Grade:
Tumor grade refers to the assessment of how abnormal cancer cells appear under a microscope compared to normal cells of the same tissue. It is a measure of the differentiation or maturity of the cancer cells. The three main grades typically used are:
- Grade 1 (Low Grade): In this grade, cancer cells closely resemble normal cells, and they tend to grow and spread slowly. These tumors are considered well-differentiated.
- Grade 2 (Intermediate Grade): Grade 2 tumors have cells that are somewhat abnormal in appearance and behavior. They grow and spread at a moderate rate and are considered moderately differentiated.
- Grade 3 (High Grade): Grade 3 tumors have highly abnormal cells that appear very different from normal cells. They tend to grow rapidly and are considered poorly differentiated or undifferentiated.
In some cases, the grading scale may be more complex, such as a numerical scale from 1 to 4 or using specific grading systems tailored to certain types of cancer. A higher grade typically indicates a more aggressive cancer with a worse prognosis.
2. Tumor Stage:
Tumor stage describes the extent and spread of cancer within the body. It is determined through a combination of clinical, imaging, and pathological assessments. The most commonly used staging system is the TNM system, which stands for Tumor, Nodes, and Metastasis:
- T (Tumor): This component assesses the size and extent of the primary tumor. T categories typically range from T0 (no evidence of a primary tumor) to T4 (a large primary tumor with significant local invasion).
- N (Nodes): N describes whether cancer has spread to nearby lymph nodes. Lymph nodes are small, bean-shaped structures that play a vital role in the immune system. The N categories range from N0 (no regional lymph node involvement) to N3 (extensive regional lymph node involvement).
- M (Metastasis): M indicates whether cancer has spread to distant organs or tissues. M0 means no distant metastasis, while M1 indicates the presence of distant metastases.
The combination of these T, N, and M categories results in an overall stage, typically expressed in Roman numerals (e.g., Stage I, Stage II, etc.). Lower stages generally indicate smaller, localized tumors with better prognoses, while higher stages suggest more advanced disease with a potentially poorer outlook.
It's important to note that the specific staging system and criteria can vary depending on the type of cancer. Accurate staging is crucial for determining appropriate treatment options and predicting a patient's prognosis.
In summary, tumor grade assesses the degree of cell abnormality and differentiation, while tumor stage assesses the extent of cancer spread within the body. Both grade and stage are critical for guiding treatment decisions and understanding a patient's prognosis in the context of cancer care.

However, oligodendrogliomas typically do not degenerate into glioblastomas.
Additional videos in the playlist will address the imaging findings of other types of brain tumors.
The level of this lecture is appropriate for radiology residents, radiology fellows, and trainees in other specialties who have an interest in neuroradiology or may see patients with brain tumors.

Growth factors: Cancer cells develop growth self sufficiency (Synthesize self responsive growth factors) Eg: a. PDGF Glioblastomas
b. TGF-a Sarcomas
c. FGF Breast ca 2. Growth factor receptors:Cancer cells encode for growth
factro receptors. a. EGF-receptor family: ERB-B1 (overexpression)
Squamous cell carcinoma
b. CSF-1 receptor: FMS (point mutation) - Leukemia c. PGDF receptor: PGDF-R (overexpression) - Gliomas 3. Signal transduction proteins - Receive growth

Eg. RAS family (point mutation) - Ca colon, lung
Pancreas
4. Nuclear regulatory proteins: Transcriptional activators Eg: C-MYC (translocated) - Burkitt lymphoma N-MYC (Amplification) - Ca lung,
5. Cell cycle regulatory proteins: Eg: Cyclin D (Translocation) Ca breast, Ca esophagus CDK4 (Amplification) Melanoma

Local invasion;
This spread is a cellular process, in which cancer cells become mobilized and invade out and away from the primary tumor site. Local invasion causes debilitating tissue and organ destruction in glioma and lung
Metastasis;
In metastasis, cancer cells break away from the original (primary) tumor, travel through the blood or lymph system, and form a new tumor in other organs or tissues of the body. The new, metastatic tumor is the same type of cancer as the primary tumor.
