Gilbert syndrome
(hyperbilirubinemia type 1, Meulengracht syndrome)
A genetic liver disorder, where the patients inherit mutated UGT1A1 gene. They do not produce enough an enzyme, uridine diphosphate glucuronosyltransferase (UGT) to keep bilirubin at a normal level. As a result, excess bilirubin builds up in the body (hyperbilirubinemia).
Many people never have symptoms. Occasionally, jaundice, fatigue, weakness, and abdominal pain may occur. Typically no treatment is needed.
(Signs and symptoms)
• Jaundice.
• Dark-colored urine and clay-colored (pale) stool.
• Flu-like symptoms (fever and chills).
• Loss of appetite, fatigue, dizziness.
• Nausea, abdominal pain, diarrhea.
(Triggers that bring the symptoms)
• Dehydration.

What is Gilbert's Syndrome? | My Mysterious Disease
Today on the Transplant Helper My Mystery Disease. What is Gilbert's Syndrome? I have suffered from a mysterious illness for over 20 years. When it first showed up I was experiencing a severe migraine. After being diagnosed with Gisbert's Disease and understanding what I was facing knowledge became a huge advantage.
Gilbert's syndrome (GS) is a mild liver disorder in which the liver does not properly process bilirubin. Many people never have symptoms. Occasionally a slight yellowish color of the skin or whites of the eyes may occur. Other possible symptoms include feeling tired, weakness, and abdominal pain.
Gilbert's syndrome is due to a mutation in the UGT1A1 gene which results in decreased activity of the bilirubin uridine diphosphate glucuronosyltransferase enzyme. It is typically i...

Gilbert’s syndrome is a genetic condition that affects the liver’s ability to process bilirubin. Although generally considered harmless, in fact patients with this condition can suffer from fatigue, gastrointestinal issues and fat soluble vitamin deficiencies.
In this episode, Nutritionist Maria Allerton (née Shaflender) talks to us in depth about Gilbert’s syndrome, speaking both from vast clinical experience and personal perspective. She discusses some of the subtle symptoms of the condition, the connections between Gilbert’s and pyroluria, why patients need to be careful when taking medications, as well as her preferred dietary and supplement interventions.

Elizabeth Gilbert was once an "unpublished diner waitress," devastated by rejection letters. And yet, in the wake of the success of 'Eat, Pray, Love,' she found herself identifying strongly with her former self. With beautiful insight, Gilbert reflects on why success can be as disorienting as failure and offers a simple -- though hard -- way to carry on, regardless of outcomes.

• Fasting or skipping meals.
• Not getting enough sleep.
• Stress.
• Overexertion.
• Menstruation.
• Illness with an infection.
• Having surgery.
(Decreased detoxification ability to some drugs in the liver)
• irinotecan: An analogue of camptothecin (topoisomerase inhibitor) used in cancer chemotherapy.
It is hydrolyzed by carboxylesterases and forms SN-38, which is metabolized via glucuronidation by UGT in the liver and excreted into the bile. Decreased UGT activity may lead to severe toxicity (diarrhea and neutropenia).
• paracetamol (acetaminophen): A medication used to treat fever and mild to moderate pain.
It is metabolized primarily in the liver, mainly via glucuronidation and sulfation (as well as via oxidation by cytochrome P450 enzymes and deacetylation), and the products are then eliminated in the urine. It can cause acute liver failure (hepatotoxicity) by overdose.

It is typically inherited in an autosomal recessive pattern and occasionally in an autosomal dominant pattern depending on the type of mutation. Episodes of jaundice may be triggered by stress such as exercise, menstruation, or not eating. Diagnosis is based on higher levels of unconjugated bilirubin in the blood without either sign of other liver problems or red blood cell breakdown.
Typically no treatment is needed. If jaundice is significant phenobarbital may be used. Gilbert's syndrome affects about 5% of people in the United States. Males are more often diagnosed than females. It is often not noticed until late childhood to early adulthood. The condition was first described in 1901 by Augustin Nicolas Gilbert.
