A fellowship is the period of medical training in the United States and Canada that a physician or dentist may undertake after completing a specialty training program (residency). During this time (usually more than one year), the physician is known as a fellow. Fellows are capable of acting as attending physician or consultant physician in the generalist field in which they were trained, such as internal medicine or pediatrics. After completing a fellowship in the relevant sub-specialty, the physician is permitted to practice without direct supervision by other physicians in that sub-specialty, such as cardiology or oncology.
Medicine (British English i/ˈmɛdsᵻn/; American English i/ˈmɛdᵻsᵻn/) is the science and practice of the diagnosis, treatment, and prevention of disease. The word medicine is derived from Latinmedicus, meaning "a physician". Medicine encompasses a variety of health care practices evolved to maintain and restore health by the prevention and treatment of illness.
Medicine has existed for thousands of years, during most of which it was an art (an area of skill and knowledge) frequently having connections to the religious and philosophical beliefs of local culture. For example, a medicine man would apply herbs and say prayers for healing, or an ancient philosopher and physician would apply bloodletting according to the theories of humorism. In recent centuries, since the advent of science, most medicine has become a combination of art and science (both basic and applied, under the umbrella of medical science). While stitching technique for sutures is an art learned through practice, the knowledge of what happens at the cellular and molecular level in the tissues being stitched arises through science.
Medicine is an open access peer-reviewedmedical journal published by Lippincott Williams & Wilkins. It was established in 1922. Of general medical journals still in publication since 1959, Medicine had the highest number of citations per paper between 1959 and 2009. The journal covers all aspects of clinical medicine. On March 31, 2014 Medicine changed into an open access publication, publishing in a broad spectrum of medical specialties.
This article deals with medicine as practiced by trained professionals from ancient times to the present. Early medical traditions include those of Babylon, China, Egypt and India. The Greeks went even further, introducing the concepts of medical diagnosis, prognosis, and advanced medical ethics. The Hippocratic Oath, still taken (although significantly changed from the original) by doctors up to today, was written in Greece in the 5th century BCE. In the medieval age, surgical practices inherited from the ancient masters were improved and then systematized in Rogerius'sThe Practice of Surgery. Universities began systematic training of physicians around the years 1220 in Italy. During the Renaissance, understanding of anatomy improved, and the microscope was invented. The germ theory of disease in the 19th century led to cures for many infectious diseases. Military doctors advanced the methods of trauma treatment and surgery. Public health measures were developed especially in the 19th century as the rapid growth of cities required systematic sanitary measures. Advanced research centers opened in the early 20th century, often connected with major hospitals. The mid-20th century was characterized by new biological treatments, such as antibiotics. These advancements, along with developments in chemistry, genetics, and lab technology (such as the x-ray) led to modern medicine. Medicine was heavily professionalized in the 20th century, and new careers opened to women as nurses (from the 1870s) and as physicians (especially after 1970). The 21st century is characterized by highly advanced research involving numerous fields of science.

Alright so it seems that there are a lot of people who have already completed their residency or may have also specialized in any given field of Medicine and want to gain some United States experience or an Advanced qualification in the US. I have known people who did take this route. One actually was in Radiology. The other in Physical Medicine and Rehab. And another in anaesthesia. It appears to my knowledge if you want to have any experience in the US you need to be ecfmg certified. That means you will need to go through the United States medical licensing examinations, the USMLE. But this will likely not be as straightforward as applying for the match such as for residency or even for a fellowship. You will have to research individual specialty programs and you can find that information on the ERAS website and look for participating programs and Specialties for Fellowship. Have to contact them directly one by one. Have your CV ready and updated. Call them and email them directly on the contact information provided on the ERAS website. Give them detailed information on your qualifications and your eagerness to be part of this Fellowship and all the skills which you can bring to the program. So basically you have to market yourself. You have to show your skills. If you have extensive research experience then that is going to be a plus and your research skills might be something that they are looking for. You have to try to get them to want to take you. And you will have to try to make it as easy for them as possible. There may be Visa issues involved. They may be some additional costs involved. They would also be extensive paperwork involved. But as you see this would be going through a process which is not normal but if you can talk to enough people and encourage them and show them that yes you are the person that they want then they may be willing to take that extra effort and get you in there. If this is something you really want to do, it will take some doing. But that's my take based on my information. But if anybody has any other ideas and experiences let me know in the comments below. Over and out.
