In this lesson I break down the basic info about our most commonly used airway management device, the endotracheal tube, or ET tube. When we need full control of our patients breathing and ventilation, the endotracheal tube is the best device to be able to deliver precise breaths. It has an interesting history, which I talk about, but there are many different parts of the tube from the tube itself, the tip, the cuff and connector, which I review over each of those parts here. In addition, I also quickly cover some of the risks and compl...

Suctioning the endotracheal tube - medical animation
Endotracheal suctioning (ETS) is a necessary practice carried out in intensive care by nurses. As shown in this medical animation, It involves the removal of pulmonary secretions from a patient with an artificial airway in place. Suctioning clears mucus and is essential for proper breathing as secretions left in the tube could become contaminated and a chest infection could develop. This is imperative if surgery is a requirement.
After intubation, the closed-suction technique is the preferred method for suctioning because the patient is connected to the ventilator throughout the suctioning procedure which facilitates continuous oxygenation. The closed-suction technique allows for a continued tidal volume delivery to the patient with minimal loss of lung volume. Closed suctioning is sugge...

Securing an Endotracheal Tube with Twill Tape and a Rolling Hitch
Jeff Ward, RRT, a respiratory therapist at Mayo Clinic, demonstrates knot tying technique for securing endotracheal tubes (ETT) to prevent their displacement or accidental extubation. He will show how the rolling hitch can be tied quickly. When used with 3/8" or ½" twill/umbilical tape, it has excellent holding force and when combined with the square knot is well suited for fixation of ETTs.

Closed suctioning is suggested for an adult patient with high fraction of inspired oxygen (FIO2) or positive end-expiratory pressure (PEEP) or for a patient who is at risk for lung derecruitment.
