merging more patches (r479:483) from 1.0-maintenance

- dhtml history now assumes all events are GET requests, not POST (RESTful compliance)
- dhtml history no longer performs useless updates
- active record permissions no longer chokes when current user hasn't been assigned yet
- 'cancel' links on create/update forms now work even after a failed validation
