Updates dependencies and makes @cloudflare/workers-types a dependency #107

Users of kv-asset-handler also need @cloudflare/workers-types, so we move it to be a dependency instead of a devDependency. Also updates ava from 1.4.1 => 3.X.X, which means we no longer need the `esm` dependency. Also removes `ts-loader`, because we're not using webpack. Other updates should be self-explanatory.
