docs: update top level markdown files

The README.md had a old reference to the CONTRIBUTING.md file, the branch no longer exists. Reference the local file instead. We're no longer on Mattermost for public chat, reference Matrix instead. The juju IRC channel is no longer monitored, remove from lists.
