Our campus community is committed to maintaining the highest level of academic and behavioral integrity. Faculty, staff, and students are encouraged to report alleged incidents of student misconduct.

Reporting Academic Misconduct: All academic misconduct at UW is reported centrally to the student conduct reporting tool below. Once the preliminary review is complete, cases may be routed to a Dean designated conduct Officer as appropriate.
Reporting Non-Academic Misconduct:All non-academic conduct concerns are managed by staff within the Division of Student Life and CSSC. Please use the form below to submit non-academic concerns of misconduct.
Students
- If you believe you have been a victim of an alleged violation of the Student Conduct Code or you are aware of an alleged violation of the Student Conduct Code, you have the right to report it to the University. You can use the online reporting form linked above to provide a statement. You will be able to directly upload text messages, social media posts, email communications, or other relevant evidence. A member of the Community Standards & Student Conduct team will contact you to review the student conduct process. The University will provide a prompt, fair, and impartial investigation to determine whether the Student Conduct Code has been violated.
- If you are reporting sex-and-gender-based violence, harassment, or discrimination, please submit a report via the UW Title IX reporting form, linked HERE.
Faculty & Staff
- If you suspect academic misconduct in a student’s work, you are encouraged to report it.
- Using the secure online form linked above, you will be able to directly upload relevant documents, such as papers with suspected plagiarism, the course syllabus, copies of exams, email communication, etc. For more examples of appropriate documents in evidence, please refer to our reporting academic misconduct page.
