PARTS AND SAMPLE CONTENT HEADING PROJECT TITLE • The title must be short, meaningful and a reflection of the expected outcome. • The project title in a proposal is your first opportunity to catch the attention of your target clientele: – Barangay – Agency – Institution – Community PROJECT DESCRIPTION
• Goal of the Project
– A goal is a long-term aim and it may not be achievable during the life of the event • Who will be the proponent of the project? • What will be the benefit from participation in the project? • Who will benefit from the project? • Specific Objectives – An objective is a short-term aim and it has to be achieved during the project period ADOPTED COMMUNITY, TARGET PARTICIPANTS AND PERSONS INVOLVE Adopted Community: San Bartolome High School, Bacolor, Pampanga
Target Participants: This training-workshop intends to get the active participation and engagement of the officers of the Recognized Academic Based Organizations, Non-Academic Based Organizations and Class Officers in San Bartolome High School.
Persons Involve: • San Bartolome HS Administrators • Adviser/s and officers of Recognized Academic and Non-Academic Based Organizations and Class Officers • DHVSU CWTS students particularly the “Gilas” and “Angat” Team consisting of four (4) sections of BS Psychology • DHVSU CWTS Implementer • Barangay Officials • Sanguniang Kabataan Officials MATERIALS AND EQUIPMENT NEEDED • Enumerate all the needed materials, equipment and logistics needed in the implementation of the project. Materials and Logistics Needed: The following paraphernalia are to be provided by the organizers: • First aid kit and bandages (to be used on the presentation and workshop) • Reading materials (Photocopies) to be distributed to the participants • Documentation (Photo printing and video editing) • Certificates Other logistics that will be provided by the implementing partners • Sound System (Will provide by the host school) • Snacks for the participants (Will provide by the SK Council) SOURCE OF FUNDING • This part will clearly state all entities who will fund the project. Also, will include various partners, donors and sponsors that will aid the project owner in the implementation of the project. Source of Funding: Major operational expenses are subsidized by the DHVSU NSTP-CWTS Angat and Gilas Team. Other miscellaneous expenses will be subsidized by partner organization and donors. TIME FRAME • 1st Saturday (8:00AM -11:00AM): Lecture on Concepts of Disaster and Emergency Preparedness • 2nd Saturday (8:00AM -11:00AM): Lecture and Workshop on Basic First Aid and Life Support • 3rd Saturday (8:00AM -11:00AM): Lecture and Workshop on Emergency Rescue and Transfer including Earthquake and Fire Evacuation Drill • 4th Saturday (8:00AM -11:00AM): Participants return demonstration and evaluation per group. • 5th Saturday (8:00AM -11:00AM): Culminating Activity SIGNATORIES
