Instruction: Please give your honest evaluation with the following statement and with its
appropriateness of each statement with the following criteria. Kindly check the number
which corresponds to your best judgments.

1. The items in the instrument are relevant to answer the objectives of

the study.
2. The vocabulary level, language structure, and conceptual level of

questions suit the level of the research participants/respondents.
3. The directions and items are constructed clearly and

comprehensively.
Presentation and Organization of Items
4. The items are presented and organized in logical manner. 
5. Questions are stated in the clear and simple terms. 
6. The instrument has an appropriate sample of items for the construct

being measured.
7. The items in the instrument are stated clearly. 
8. The items and their alternatives are neither too narrow nor limited in

its content.
Sustainability of Items
9. The items are appropriate and represent the substance of the

research.
10. The items on the instrument can elicit responses which are stable,

definite, consistent, and not conflicting.
11. The questions are sufficient enough to give answers to the

objectives of the study.
Adequateness of Content
12. The items adequately represent the coverage of the research. 
13. The number of questions per research questions is represented

enough of all the questions needed for the research.
Research Participant/Respondent Friendliness
14. The questionnaire creates positive impression, one that motivate

the respondents to answer them.
Objectivity
15. No aspect of the questionnaire suggest bias on the part of the

researcher.

The contents of the rating scale are generally structured in a positive language,
albeit few items contain negative words which, if necessary, should be revised. Contents
have reasonable length and not hard to understand.
