to participate in this youth event that requires transportation to a location away from
our barangay. This activity will take place under the guidance and direction of LGU –
Kitaotao and Local Youth Development Office. I understand that participation on this
youth event involves certain inherent risks and regardless of the precautions taken by
LGU Kitaotao, some injuries may occur. I understand that LGU Kitaotao is held
accountable and responsible for my safety during the event only which will happen
from August 5, 2024 8:00 AM to August 8, 2024 5:00 PM.
LGU Kitaotao is not accountable and responsible for the days that were not
mentioned above. I agree to comply with the LGU Kitaotao’s customary terms and
conditions for participation. I agree to follow these rules as well as any others that
may be given by the LGU. I am voluntarily requesting permission for my son/daughter
to participate.
