1.0 PROJECT INTRODUCTIONS

1.1 Project Details

Project Title : TERM OF CONTRACT PROPOSED REPAIR EXPANSION JOINT BRIDGES


AT VARIOUS LOCATION, SRI AMAN DIVISION ON “AS AND WHEN
REQUIRED BASIS”.

Client : Division Engineer, Sri Aman Division

Main Contractor : Pembinaan Tenaga Bumi Sdn Bhd

Project Duration : 12 months

2.0 MANAGEMENT RESPONSOBILITIES

2.1 Project Director / Managing Director


Project Director / Managing Director shall be responsible for the overall operation and effectiveness of
Project Safety Plan. PD responsibilities include:
- Managing the Project Safety Plan
- Training and coordinating of personnel in Project Safety Plan
- Development of the auditing program
- Co-ordination management reviews of the plan
- Following up observation/findings from management reviews, audits and customer reviews
- Assisting personnel during audits of the plan by third party certification bodies and customers

2.2 Project Manager


Project Manager responsibilities include:
- Managing of the operation department
- Assist in design, develop and monitor Safety and Health Policy
- To regularly review and update the Safety and Health Policy and procedures to meet client’s
requirement
- To run the operation in a safe manner

- To implement and review all aspects of the Project Safety Plan


- To chair and coordinate Safety and Health Committee meetings

2.3 Safety and Health Officer


- Support the development of OHS policies and programs
- Advise and instruct on various safety-related topics (noise levels, use of machinery etc.)
- Conduct risk assessment and enforce preventative measures
- Review existing policies and measures and update according to legislation
- Initiate and organize OHS training of employees and executives
- Inspect premises and the work of personnel to identify issues or non-conformity (e.g. not using
protective equipment)
- Oversee installations, maintenance, disposal of substances etc.
- Stop any unsafe acts or processes that seem dangerous or unhealthy
- Record and investigate incidents to determine causes and handle worker’s compensation claims
- Prepare reports on occurrences and provide statistical information to upper management

2.4 Engineer
Engineer responsibility is to control the construction work force. The virtue of his position, he is a member
of the project Safety and Health Committee. His duties and responsibilities are as follow:
- Cooperate with the Project Management to ensure the successful implementation of the Project
Safety Plan.
- Ensure site activities under their charge at all times comply to acceptable safety and health standards
and according to Project Safety Plan.
- Take corrective actions to identify and eliminate hazardous work conditions and practices.
- Ensure the project site is well organized and tidy to prevent accident.
- Assist in the enforcement of site safety and health regulations.

2.5 Site Safety Supervisor


Site Safety Supervisor (SSS) remain assisting SHO on the responsible for the aspects of safety and health at
site. SSS will carry out safety toolbox briefing on safety requirement on site including maintain a daily safety
log record:

- Personnel Protective Equipment (PPE)
- Any safety and health problem arising from work and proposed recommendation
- Report all incident or accident to SHO
- They will ensure that the team workers are fully aware the stringent requirement of safety on site
constantly

2.6 Site Supervisor


All supervisor is responsible to the Construction Manager for the safety of personnel under their control.
He/she is responsible for the site personnel under his supervision and accountable for the effective
enforcement of the safety and health activities within his/her area of responsibility. This includes:
- Maintain safe working condition and good housekeeping of all areas under his jurisdiction
- Enforce the use and inspection of personnel protective equipment such as hard hats, eye protection,
appropriate gloves, safety shoes, harness, etc as required under OSHA 1994
- To correct any unsafe work practice of workers and any unsafe condition when observed or noted
- Report to Project Engineer and SHO on all accident/incident, near misses, dangerous occurrence,
occupational poisoning, and occupational diseases and to assist in investigation
- Continuously monitor the work area where working condition are likely to cause personnel injury or
property damage and immediate safety measure imposed to rectify the unsafe condition
- Enforce all safety rules and plan safety into all daily work activities
- Ensure the job hazard and safety procedures are explained to each site personnel before work begin
by conduct daily safety toolbox briefing with proper record keeping
- Work in close co-operation with SSS in the elimination of unsafe practices and hazardous conditions
at the worksite

4.0 HAZARD IDENTIFICATION, RISK ASSESSMENT AND RISK CONTROL (HIRARC)

4.1 Term and Definition

Hazard is a source or a situation with the potential for harm in terms of human injury or ill-health, damage
to property, damage to the environment, or a combination of these.

Hazard Control means the process of implementing measures to reduce the risk associated with a hazard.

Hierarchy of Control means the established priority order for types of remedial satisfy measures to be used
to control risks.
Hazard Identification means the identification of undesired events that lead to the realization of the hazard
and the mechanism by which those undesired events could occur.
Risk means a combination of the likelihood of an occurrence of a hazardous event with specified period or
in specified circumstances and the severity of injury or damage to the health of people, property,
environment, or any combination of these caused by the event
(Risk = Likelihood x Severity)

Risk Assessment means the process of evaluating the risks to safety and health arising from hazards at work.

Risk Assessment Team is responsible for conducting risk assessment of the work activities at their workplace.

Risk Appetite is the amount and type of risk that an organization is prepared to pursue, retain or accept.

Risk Management means the total procedure associated with identifying a hazard, assessing the risk, putting
in place control measures and reviewing the outcomes.

Risk Assessment Register means the collecting of risk assessment within the organization.

4.2.4 Action for risk level

Risk Severity Description


A HIGH risk requires IMMEDIATE action to control the hazard as
15-25 HIGH detailed in the hierarchy of control. Action taken must be documented
on the risk assessment form including the data for completion.
A MEDIUM risk requires a planned approach to control the hazard and
applies temporary measures if required. Actions taken must be
5-12 MEDIUM
documented on the risk assessment form including the date of
completion.
A risk identified as LOW may be considered as acceptable and further
reduction may not be necessary. However, if the risk can be resolved

quickly and effeciently, control measures should be implemented and
recorded.

6.3.1 Receiving Authority

The position of Receiving Authority is the responsibility of the Engineer who is in charge of the scope of work
to be executed. The Engineer shall complete the description of the work to be carried out supplying all
relevant information inclusive of drawings method statements and risk assessments in support of the permit
application.

The Receiving Authority of the Work Permit has the following responsibilities:
- To instruct the Performing Authority (Performer) of the permit requirements and satisfy
herself/himself that the Performing Authority has enough information and resources to perform the
work safely.

- Ensures that the Performing Authority understands what, where, and how the work is to be
performed.
- To ensure compliance with the conditions stipulated on the permit.
- To ensure that the Performing Authority supervises the work group and is satisfied that the persons
carrying out the work use the proper tools and equipment for the job.
- Performing Authority informs the work group of the safe working practices.
- To inform Issuing Authority of any changes in the condition of the job or the surrounding area, or of
any delay in starting the work, so as not to infringe upon any safety factors.

6.3.2 Issuing Authority (Validator)

Engineer shall ensure that there are sufficient signatory’s onsite to fulfil the role of approving the Permit of
Work. This role shall be held by an employee with the minimum management level of Superintendent and
shall sign the Permit of Work as Issuing Authority. He/she will be the responsibility of the Issuing Authority.

The engineer may designate persons other than herself/himself to approve Permit to Work. This designation
will be made in writing and informed to the Contractor Field Safety Supervisor/Officer and all relevant permit
signatories.
The Validator of the Permit to Work has the following responsibilities:
- Check the completeness and reliability of the submitted forms
- Specify the required worksite precautions and controls
- Detail required safety equipment
- Identify any worksite preparation requirements, if deemed necessary
- Ensure that Certificates are duly signed off by authorized engineers
- Ensure that permits are authorized on the day of use
- Ensure that permits are closed on the day of use

No work can commence without the approving authority’s signature. In this context engineer on her/his
designee has the responsibility of approving the work to commence and shall sign the Permit to Work as
Approving Authority.

6.3.3 Operating Authority

The Operating Authority is responsible for carrying out all preliminary checks required by Issuing Authority
and will sign the Permit to Work to verify that all requirements have been met and that the work area is in
safe condition for the work to commence.

The Operating Authority is by Project Manager. Additionally, the Operating Authority shall:
- Ensure that all signatories have signed the Permit to Work
- Ensure that all instructions have been given to the work party prior to the work commencing
- Inform Issuing Authority that he works is about to commence in order that this can be notified and
recorded as a permit
- Inspect the worksite periodically to ensure there is no change in condition
- Suspend the work immediately if there is a breach of this procedure or change in conditions and to
notify Issuing Authority for the reason.
- Ensure a good standard of housekeeping is maintained throughout the work and that all equipment
is removed upon completion

- Notify Issuing Authority upon the completion of work


- Inform Issuing Authority of any change in the working conditions of the surrounding areas in the
vicinity of the working area.

6.3.4 Performing Authority (Performer)

The Performing Authority is the person responsible for supervising the work carry out and shall hold as a
minimum the supervisory level of Foreman. The Performing Authority is not entitled to start any job without
approval of the Permit to Work by both Issuing Authority and Operating Authority. Performing Authority is
responsible to:
- Ensure the work carry out is fully briefed by means of dedicated toolbox briefing
- Ensure that they have clearly understood instructions given, including the scope of their activity and
the relevant risks
- Remain at the worksite until the completion of the task

After signing g the Permit to Work the Performing Authority shall ensure that all copies of the Permit to
Work are correctly distributed and that a copy is retained and posted at the worksite. In addition, Performing
Authority shall:
- Inform the Receiving Authority or Operating Authority of any change in the working conditions of the
job, or a delay in starting the work
- Using proper tools and equipment for the safe execution of the job
- Ensure the worksite is left in a safe and tidy condition

6.3.5 Permit Closure

The Permit to Work will be considered to be closed when Issuing Authority signs the completion section of
the Permit to Work (after receiving the subcontractor conformation on the Permit to Work that the work is
complete). Acceptance of this signature is conditioned to:
- Completion of the work according to the instructions
- Cleanliness of work area, including that removal of all the work equipment
- Re-installation of all safety conditions

7.0 FIRE PRECAUTION AND PROTECTION AND EMERGENCY RESPOND

Fire occurrence within the project site can be from many sources including the surrounding forest which is
quite common during a dry spell. The occurrence and severity of forest fire is very much dependant on the
prevailing weather conditions, particularly when there is a prolonged dry spell.

7.1 Fire Precaution


- Strict enforcement of “NO SMOKING” practice at construction workstation and workers camps
- Located designated smoking areas at site
- Flammable materials shall be stored at a safe distance and far away from sources of ignition
- Open burning of construction waste must be strictly prohibited

- Firebreaks or buffers shall be established around fire potential areas such as the hazardous material
storage yard to prevent forest fire encroaching the yard and vice-versa
- Waste disposal sites shall be frequently monitored for possible fire occurring
- Firefighting equipment such as fire extinguishers shall be maintained in operating condition and must
be available at fire hazard areas including the workers camps
- In-house training shall conduct for all site workers no how-to response to small fires
- Fire drills shall be conduct from time to time

7.2 Fire Protection

In the event of a fire, the priority should be to put out the fire, if it is not out of control, failing which
evacuation becomes necessary. The following procedures shall be implemented if the fire is out of control:
- The ERT shall request the Project Manager to come to the scene to assess the situation
- The ERT members shall report to the Project Manager on the situation of the fire, both in terms actual
and potential size
- Project Manager shall co-ordinate and direct with the response team
- Project Manager shall immediately establish perimeter control and maintain ingress and egress
routes for emergency vehicles
- ERT personnel shall ensure all personnel must be evacuated and assembled at designated points,
preferable as far as possible from the fire. Where fire is still small and can be controlled, it shall be
the responsibility of the response team to put out the fire without delay
- Where the fire is out of control, immediate get assistance from the nearest Fire Rescue Department
- Action to prevent the spread of fire to adjacent areas or facilities must be immediately carried out
such as the provision or firebreaks or buffers

7.3.2 Emergency Response Training

Responding to contingency depends very much on the capability of the existing manpower and resources.
In view of the diversity of accidents, contingency response personnel are expected to respond effectively
when a contingency situation arises. Among the key factors for a rapid response to a contingency situation
include the effectiveness of the ERP and personnel trained to execute the plan.

It is crucial that the ERT members are familiar with the ERP. This can be achieved by having training program
in place and frequent drills conducted based on credible emergency scenarios. Frequent drills could develop
confident, build rapport with other participating agencies, and highlight weaknesses if any in plan. All
personnel involved in the contingency response team should be trained on the following:
- Firefighting
- Spill containment
- First aid and rescue

7.3.3 Emergency / Fire Drill

The ERP shall be tested at least once a year or within such frequency as the management may decide by
main contractor together with the cub-contractor in conjunction with SO/SOR advise. The involvement of
external such as BOMBA, POLICE and AMBULANCE in the dire drill shall only be carry out with the consent
of the SO/SOR of the project.

The drill shall be carried out using per-planed procedures in accordance with this ERP. The objective of the
drill is to determine the effectiveness and capabilities of the ERT in handling the emergency. The adequacy
of emergency resources, readiness of response team and sub-contractor shall be among the criteria to be
tested.

The results of the drills shall be documented and recorded for the review by the top management of
Majestika Sdn Bhd.

7.3.4 Emergency Evacuation

a. Upon notification of the evacuation call, all personnel must immediately prepare to leave the
affected area / worksite and shut down electrical appliances, equipment and machinery
b. Follow instruction by ERT personnel and leave the area by the safest escape route
c. Assist and persons with minor injuries or disabilities to leave the area or to the designated Assembly
Area
d. Walk quickly and calmly to the designated Assembly Area as advised by the respective supervisors /
emergency warden
e. All site personnel to assemble according to their work trade
f. Any missing persons shall be identified, and search and rescue teams should be sent to search for the
missing persons if the situation permits them to do so
g. First aider with ERT shall be on standby to render first aid treatment when required.
h. Remain at the assembly point until instructed to leave by the emergency response leader have issued
the clear order

8.0 SAFETY OPERATION PROCEDURES

8.1 Site Clearing


a. Felling of trees should be done by trained personnel to safeguard public safety and to prevent
damage to public property
b. Care should be taken to protect existing underground and overhead utilities (if any) especially electric
cables to avoid any undue accident during site clearing work if relation of such services is not possible.
c. Machinery operator shall be properly instructed in the work he is to do.
d. Only competent operator to operate the machinery.
e. Heavy machinery daily check by the operator and fill-up the machinery logbook.
f. Safety site inspection by SHO / SSS.
g. Means of access and egress roads at the site must be well maintained.
h. Do not horseplay on the job.
i. Do not spill machinery hydraulic oil on the ground.
j. Machinery operator shall be protected from falling objects.
k. Report any unsafe act and condition to your supervisor.

8.2 Earthworks / Excavation


a. The excavator should not be loaded beyond its safe working load.
b. In situations where the operator of the excavator does not have a clear and unrestricted view of the
attachment during work, a signalman should be made available to give signals to the operator.
c. During excavating with the excavator, the excavator and the position at which spoils are dumped
should be at sufficient distances away from the excavation edge.
d. Never left the excavator unattended with the machine ON.
e. The excavator should not cause any obstruction to the traffic, traffic signs, the view of other vehicle
drivers or other site activities.
f. When thunderstorm or lightning, no one should stay on an excavator.
g. Is trained and competent to operate an excavator.
h. Has the ability to understand and follow instruction and information contained in the manufacturer’s
manual concerning safe operation of the machine.
i. All public walkways, sidewalks and the thoroughfares bordering on or running through any
excavation site shall be provided with substantial guard-rails or board fences.
j. In every excavation of more than 1.2 meters deep there shall be provided ladders, stairway or ramps
to furnish safe access and egress from such excavation.
k. “Penyendengan”
To excavate a trench without a buffer, the sides must be cut to form of a safe slope refer to Figure
8.1 below; so that the material on the sides can withstand under working conditions to prevent soil
fall into trench area.

l. “Penanggaan”
Penanggaan is forming a series of ladders on the vertical wall of the excavation to reduce the height
of the wall and ensure stability, refer Figure 8.2. this method can be used to prevent landslides by
excavating the sides of the excavation area to form one or more horizontal levels or stairs with a
vertical surface between the levels. The vertical distance shall not exceed 1.2 meters and shall not be
less than 1.2 meters wide.

m. Combined “Penyendengan” and “Penanggaan”


In some cases, it may be appropriate to use a combination of the two methods (Penyendengan and
Penanggaan) in one excavation; Figure 8.3.

8.3 Piling
a. Each member of the pile-driving crew shall be properly instructed in the work he is to do.
b. Only competent piling rig operator to operate the machinery.
c. All pile-driving equipment shall be inspected daily by a designated person before the start of work
and every defeat shall be immediately corrected before pile-driving commences.
d. Reasonably practicable measures shall be taken to warn persons not to approach within 50 meters
of a pile under test.

e. Before placing or advancing a pile driver, the ground shall be inspected by a designated person and,
where necessary for firm and level footing, timber shall be placed. After placing or advancing a pile
driver, inspection and correction of the footing shall be made, when necessary, to main stability.
f. All mechanical equipment and plant should be well maintained throughout the piling work.
g. Power / hand tools daily check by the operator and fill-up the machinery logbook.
h. Heavy machinery daily check by the operator and fill-up the machinery logbook.
i. Safety site inspection by SHO / SSS.
j. Do not horseplay on the job.
k. Do not spill machinery hydraulic oil on the ground.
l. Piles shall be stored and stacked in a safe and orderly manner.
m. Machinery operator shall be protected from falling objects.
n. During the hoisting of piles, all persons not actually engaged in operating the equipment and handling
of piles shall be kept out of the area.
o. Report any unsafe act and condition to your supervisor.

8.4 Lifting
a. Lifting machinery with valid certificate of fitness.
b. Competent operator with valid competency from DOSH. The operator shall be familiar with the
cranes, its care, the operator’s manual and the load charts. Upon request, the operator shall
demonstrate his/her ability to determine total load weight and its relationship to the crane and charts.
c. The operator shall have final responsibility and control over the crane operations. Whenever there is
any doubt as to safety, the operator shall have the authority to refuse to handle a load until
corrections for safety are completed.
d. For mobile crane, the following steps should be followed to ensure the stability of the mobile cranes
during the operation:
- All outriggers should be fully extended.
- All jacks should be placed upon adequate footings and packing. Care should be taken to
ensure that there is adequate bearing surface under the packing, especially where the ground
is soft or backfilled.
- The crane should be levelled and all outriggers beam packed in case of rebound, close to the
ends of the outrigger beams. Anchor pins should be inserted where provided in chassis and
outriggers.
- The base timbers of any packing, i.e. those timbers resting on the ground, should be closely
packed, and should cover as large area as necessary to safely transmit their load to the ground.
- All timber packing should be sound, free from defects, of ample and adequate strength.
- The base layer packing should be of sufficient length, width and thickness to cover the area
required. These should be closely laid over a levelled and consolidated area.
- Packing should not be removed until all operations are completed.
e. Slinger and rigger should have adequate ability, knowledge or authority.
f. While panels or structural members are being hoisted, tag lines shall be used to prevent uncontrolled
movement.
g. Loads shall be attached to the crane hook by means of slings, shackles, or other approved devices.
Hooks used for lifts shall have safety latches or safety wires to prevent slings from coming off hook.

h. No lifting of load should be carried out outside the hoarded or fenced up area of the worksite. If
lifting operation were to be carried out outside the hoarded or fenced up area of the worksite,
written approval from the local authorities shall be obtained. Working area should be cordoned off
and means for public control such as setting up of warning signs, warning lights to direct public or
traffic away from it and controlled by a designated person shall be provided.
i. Danger zone should be established before any lifting operation is carried out. The perimeter of the
danger zone must be cordoned off. If danger zone extends beyond the worksite hoarding or fenced
up area, means for public control such as setting up of warning signs, warning lights to direct public
or traffic away from it and controlled by a designated person shall be provided.

8.5 Concreting Works


a. Formwork and reshoring shall be certified structurally safe by a Professional Engineer and shall be
properly braced or tied together so as to maintain position and shape.
b. The formwork including the shores, braces and other supports shall be inspected by the designated
person:-
- During erection
- During and after concreting
- After any event likely to have affected their stability, for example after heavy rain or following
strong wind
c. All workers involved in the work should be fully equipped with the necessary PPE.
d. To ensure that a good foundation is established while handling a vibrator engine.
e. While carrying out concreting by the roadside, concrete trucks are to be parked as near as possible
by the roadside.
f. Extra concrete overflowing from the mixer on the road surface should be cleared immediately so as
not cause hardened lumps.
g. All concreting equipment and mold shall be kept safe and stored in an area away from structure edge.

8.6 Scaffolds
When scaffolding is to be used, the following must be ensured:-
a. Every scaffolds and every part thereof shall be of good construction, of suitable and sound material
and of adequate strength for the purpose for which it is used.
b. Every scaffold and every part thereof shall comply, with the Malaysian Standard or any other relevant
international standard.
c. Metal parts used for scaffolds shall be of suitable quality and be in good condition and free from
corrosion or other patent effect likely to affect their strength materially.
d. All scaffolds and their supports shall be capable of supporting the load they are designed to carry
with a safety factor of at least four.
e. Every scaffold shall be properly maintained, and every part thereof shall be kept so fixed, secured or
placed in position as to prevent, as far as is practicable, accidental displacement.
f. No scaffold or part thereof shall be partly dismantled and allowed to remain in such a condition that
it is capable of being used unless the scaffold continues to comply with the safety legislation under
Factories and Machinery (BOWEC)(Safety) Regulation 1986.

g. Every erection of scaffolds exceeding 15 meters in height shall be constructed in accordance with the
design.
h. A copy of the scaffold design and drawings shall be submitted to DOSH for his records prior to be
erection of scaffolds.
i. All scaffolds materials to be used for the construction of scaffolds shall be inspected by a designated
person on each occasion before being used.
j. Every scaffold shall be securely supported or suspended and where necessary sufficiently and
properly strutted or braced to ensure stability.
k. Scaffold erector should always adopt a safe system of work during the erection, altering and
dismantling of scaffolds. This will usually include the use of fall arrest equipment.
l. All scaffolds require bracing to help prevent them from collapsing. All scaffolds, including
independent scaffolds should be secured tied, or otherwise supported. More ties will be required if:-
- The scaffold is sheeted or netted due to the increased wind loading;
- It is used as a loading platform for materials or equipment;

m. The erection of scaffold and peripheral netting should be one floor higher than the structural floor
under construction to ensure there is no chance of objects falling off the perimeter of building.
n. The details drawing of the scaffolds design will be submitted by Professional Engineer on the
separated document.

8.7 Heavy Machinery


a. All machinery and vehicles intended to be used for the purpose of construction within the project
site must be in possession of valid licenses and permits.
b. Machinery and vehicles are to be always maintained in good order and road worthiness. Owner or
authorizes persons in charge of machinery and vehicles are to ensure that these machines are always
in safe operating condition. Records of maintenance and repairs shall be kept.
c. Drivers and operators shall regularly inspect the machines and vehicles being used and shall be
responsible for reporting any defects to the assigned person in charge. Any defects shall be
immediately rectified before use.
d. Driver shall comply with the permissible speed limit at the site.
e. Work at a safe distance from all power lines / poles at least 10 feet away with voltages up to 50kV.

8.8 Gas Cutting and Welding


The following general requirement are to be followed whilst carrying out gas cutting burning and welding.

8.8.1 Gas Cutting


a. A standard coloring of hose lines should be incorporated, red for acetylene or fuel gas, green or black
for oxygen.
b. Hoses must be filled with flashback arrestors.
c. Regular inspections by a competent person must be carried on regulators, hoses and cutting torches.
d. Hoses must be properly laid to avoid tripping hazard and clear of sharp edges, failing objects or sparks.
e. Only sparks lighters are to be used for lighting torches. Use of disposable gas lighters is strictly
prohibited as this has been proven to cause explosions and to be a fire hazard.

f. Always make it a practice to close valve at the bottle whenever breaking off for a long period of time.
g. Gas cylinders should not be dropped, slid or rolled about. They should always be kept in upright
position and properly secured or caged. Empty cylinders should be marked.
h. Cylinders should be placed away from source of heat.
i. Appropriate PPE must be worn whilst cutting and burning.
j. Do not use in the vicinity of inflammable materials.

8.8.2 Welding
a. Appropriate PPE including filtered face shield, long sleeve jacket, leather gloves and boot must be
worn.
b. Earth cable to be firmly secured.
c. Electrode holder must be properly insulated, and electrode removed from the holder when not in
use.
d. Welding should be well insulated and good condition and properly laid to avoid damage of able or
tripping hazards.
e. Screens must be used to protect persons nearby from the sparks.
f. Welding machine must be provided with an approved type of a portable fire extinguisher.

8.9 Use of Generator and Air Compressor


8.9.1 Generator
a. Only competent personnel are allowed to operate or maintain a generator.
b. Lubrication, water level, fuel should be checked before operating.
c. Regular inspection and maintenance to be carried out. Generator should be properly earthed, and
the termination point checked.
d. Do not attempt to repair defective equipment. It should only be repaired by an authorized electrician.
e. Extra care should be taken when operating any equipment in wet conditions.
f. All exposed electrical wires, damage conduits; plug of switches must not be used until fully rectified.
g. Switch of the main power supply whenever equipment is not used.

8.9.2 Air Compressors


a. All air compressors should have valid PMT issued by DOSH.
b. Compressors shall not be operated by incompetent persons.
c. Regular inspection and maintenance are required.
d. Do not use compressed to remove dust from cloth or cooling the body.
e. Air manifolds and connections must be regularly maintained to prevent leakage.
f. Ensure that the air hose is suitable for its intended pressure. Proper connections should be used to
join the hoses.
g. Hose couplings to be properly secured before operating or disconnecting the air hose. Pressure from
the hose may cause the coupling to shoot out and cause bodily harm.
h. Compressors shall not leave running over long period to prevent overheating.
i. Air compressors should be equipped with an approved type of portable fire extinguisher.

8.10 Electrical Safety


a. Before work is begun, the employer shall determine by inquiry / direct observation / by instruments,
where any part of an electrical power circuit, exposed or concealed, is so located that the
performance of the work may bring any person, tool or machine into physical or electrical contact
with it.
b. All conductors which may give rise to damage must be insulated, protected, placed and other
precautions take to prevent damage,
c. All wiring shall be supported on proper and not looped over nails or brackets.
d. No wiring shall be left on the ground or the floor of a building unless it is unavoidable and it is
necessary to lay electric wiring on the ground or the floor of a building, the wiring shall be of the
weatherproof types and shall be provided with adequate mechanical protection to withstand the
wear and teat to which it may be subjected and it shall be maintained in good and safe working order.
e. Where electrical appliances and current – carrying equipment have provisions made for earthing,
they shall be properly earthed.

9.0 INVESTIGATION AND INCIDENT REPORT


Incident investigation is a process for reporting, tracking, and investigating incidents that includes:-
a. A formal process for investigating incidents, including staffing, performing, documenting, and
tracking investigations of process safety incidents.
b. The trending of incident and incident investigation data to identify recurring incidents. This process
also manages the resolution and documentation of recommendations generated by the
investigations.
c. Investigation should be conducted in an open and positive atmosphere that encourages the
witnesses to talk freely. The primary objective is to ascertain the facts and roof causes with a view to
prevent future and possible more serious occurrences.
d. Accidents and dangerous occurrences that result in death, serious injury or serious damage will be
investigated by the Majestika Sdn Bhd immediately to find out the cause of the accident / occurrence
so that effective control measures can be implemented to prevent any recurrence.

9.1 Action to be Taken


a. It is important after any accident or incident, the information relating to the incident is gathered in
an organizes way. The following shall be taken:
- Take photographs and make sketches
- Examine involved equipment, work piece or material and the environment condition
- Interview the injured, eyewitnesses and other involved parties
- Identify the specific agency / sub-contractor involved
b. Carry out analysis of incident
- Establish the chain of events leading to the accident or incident
- Consider all possible causes and the interaction of different factors that led up to the accident
and identify the most probable cause
c. The next stage will be to proceed with the follow-up action
- Report on the findings and conclusions
- Formulate preventive measures to avoid recurrence

12.0 SAFETY, HEALTH AND WELFARE

Every worker of a construction site is entitled to welfare facilities provided by his employer (Majestika Sdn
Bhd / subcontractor). Welfare is a necessity for workers and required by the law. The employer should
consider at the planning and preparation phases the availability of welfare facilities, their location on site
and how they will be maintained.

Sufficient and well-maintained facilities can have a desirable benefit on health and wellbeing of workers.
Good facilities can also help prevent several occupational diseases such as Hepatitis A, Cholera and
Dermatitis.

12.1 Toilets
a. Suitable and sufficient toilets must be provided or made available at readily accessible places.
b. Toilets must be arranged for males as follows:
- Where 25 males or less: one water closet or latrine and four feet of urinal.
- Where more than 25 males but less than 50: two water closets or latrines and eight feet of urinal.
- Where more than 50 males but less than 100: three water closets or latrines and 13 feet of urinal.

c. Toilet must be arranged for females as follows:
- Where 20 females or less: one water closet.
- Where more than 50 females: one water closet and one additional water closet is provided for every
20 males in excess.
d. Closets and urinals in the interior of buildings must be of the water-flush type.
e. So far as is reasonably practicable, toilet must be adequately ventilated.
f. So far as is reasonably practicable, toilets must keep in a clean.
g. Separate rooms containing toilets must be provided for men and women.

12.2 Washing Facilities


a. Suitable and sufficient washing facilities including showers if required by the nature of the work or
for health reason, must so far as is reasonably practicable, be provided or made available at readily
accessible places.
b. Washing facility must comprise at least one wash-hand basin or equivalent trough of ample size,
having a smooth impervious surface and fit with a waste pipe and plug for every 20 or part 50 persons
employed at any one time.
c. Washing facilities must be provided:
- In the immediate vicinity of every toilet, whether also provided elsewhere.
d. Washing facilities must include:
- A supply of clean water
- Soap or other suitable means of cleaning
e. Rooms containing washing facilities must be sufficiently ventilated.
f. Washing facilities and the rooms containing must be kept in a clean or orderly condition.

12.3 Facilities for Rest


a. Suitable and sufficient rest rooms or rest areas must be provided or made available at readily
accessible places.
b. Rest areas must:
- Be equipped with an adequate number of tables and seating for the number of persons at work likely
to use them at any one time.
- Include suitable arrangement to ensure that meals can be prepared and eaten.
- Be maintained at an appropriate temperature.

14.0 TRAFFIC CONTROL AT CONSTRUCTION SITE

The traffic control at construction site designed to help drivers reduce the probability of vehicle-related
accidents happening. Employers have legal duty to ensure that the safety and health of their employees,
contractors, and members of the public.

14.1 Traffic Control


a. All vehicles used at worksites shall be roadworthy and comply with the requirements of the Road
Transport Department of Malaysia.
b. No person shall drive a vehicle of any class or description in a construction worksite unless he is the
holder of a driving license authorizing him to drive a vehicle of that class or description
c. Whenever any work is being performed over, or in close proximity to a highway or any other place
where movement of vehicular traffic into and out of the worksite may cause danger to the public,
the working area shall be barricaded. Suitable and sufficient warning signs and warning lights shall
be set up to a direct traffic to slow down or away from it, and when necessary, the traffic shall
specially be controlled by designated person.
d. Vehicles should be parked in designated areas within the hoarded area while being loaded or
unloaded.
e. Where it is authorized by the local authority for vehicles ferrying materials to the worksite to be
parked outside the hoarded area, suitable safety measures should be taken. Such measures should
include cordoning off such parking areas and suitable warning signs, lights and flagman should be
provided.
f. Where it is necessary for construction machinery to carry out work from outside the hoarded area,
the operating area of the machine outside the hoarded area should be cordoned off. Suitable warning
signs and flagman should be provided.
g. Vehicles for carrying building materials, debris and excavated materials should be clean, well
maintained and in good running condition. If they carry loose materials, they should be covered and
properly sealed to ensure that there will be no spillage of materials onto the public.
h. In residential areas, heavy vehicles engines should not be left idling unnecessarily so as to prevent a
nuisance to the public at night and during public holidays.
i. Vehicles should not be driven at speed exceeding the authorized speed limits.
j. Flagman at site to direct / guide drives on the traffic movement.

15.0 PERSONNEL PROTECTIVE EQUIPMENT (PPE)

PPE should not be used as a substitute for other methods of risk control. It should always be regarded as a
