PARENT’S CONSENT

I hereby willingly and voluntarily give consent on the participation of my


son/daughter_____________________________________________ to their practice in the
School Marching Band this coming Nov. 18, 2023, Saturday @ 8am to 4:30 pm at Punta
National High School Covered Court in preparation for the upcoming parade in connection
for the INTRAMURAL MEET 2023 this coming Nov. 28-30, 2023
I have considered the benefits that my son/daughter will derive from his/her
participation on the said practice, provided that due care and protection will be observed to
ensure the comfort and safety of my son/daughter, and that DepEd employees and personnel
may not be held responsible for any untoward incident that may happen beyond their control.
