PETITION
Petitioner, EDRACEL C. DEL ROSARIO, to this Honorable Court, respectfully
states that:

1. His date of birth is December 18, 1990, He is 32 years of age and a Filipino
citizen and is presently residing at P2-A, Remorin Subd., Village 2,
Libertad, Butuan City and residing therein since 2005 with residential
telephone no. (085)816-0050;

2. He is working as a government lawyer in the City Government of Butuan,


with an office located at Sangguniang Panlungsod Building, City Hall
Complex, J. Rosales Avenue, Brgy. Doongan, Butuan City. He was granted
by the Office of the City Mayor with an authority to practice his profession;

3. He has satisfactorily completed the required course in law at De La Salle


University College of Law in 2021;

4. He passed the 2020/2021 Bar Examinations and was admitted to practice


law in the Philippines on May 06, 2022, with roll no. 76908.  Attached is a
Copy of the Certificate of Bar Membership as Annex “A” made an integral
part of this petition;

5. He is a member of the Philippine Bar in good standing with Certificate of


Clearances from the Office of the Bar Confidant of the Supreme Court and
the Integrated Bar of the Philippines issued by the National and the Agusan
del Norte Chapter. Attached is a copy of said clearances as Annex “B” and
Annex “C to C-2”, respectively and made an integral part of this petition;
6. The undersigned has not been charged nor convicted in the first instance of
any crime involving Moral Turpitude. Attached is his NBI clearance as
Annex D;

7. He has paid his IBP annual membership fee and Professional tax for the year
2023, as evidenced, respectively, by Official Receipt No. 288293 dated 16
January 2023 issued by IBP National Office, and Official Receipt No.
2344565 issued by the Office of the Treasurer of the City of Butuan, hereto
attached as Annex “E” and “F”;

8. He desires to be commissioned as Notary Public for and in the city of


Butuan and possesses all the qualifications and none of the disqualifications
for said office and he is not commissioned as Notary Public in any other
jurisdiction;

9. Petitioner undertakes to seasonably submit his notarial reports, once his


commission as such Notary Public in Butuan City has been approved;

10.In compliance with the requirements of the Honorable Court for Notarial
Commission, attached hereto are the following annexes and made an integral
part of this petition:

a. 2x2 Picture, Three Specimen of his Official Signature and notarial


seal as Annex “G”;

b. Photo of the office desk that the undersigned will use in doing notarial
activities. Attached as Annex “H”.

PRAYER

WHEREFORE, premises considered, it is most respectfully prayed of


this Honorable Court that Petitioner be commissioned as a Notary Public in the
City of Butuan.
