To whom it may concern,

Please excuse my daughter, Shermel Trinity Duka, for failing to attend


school last week, April 18-20, she was suffering from Urinary Track Disease
(UTI) and need to stay home for her medication.

She is feeling much better now and able to attend school this week. I am
sending her to school to prevent further loss in her studies. Please guide
her for the things she has missed.
