SURVEY QUESTIONNAIRE

This questionnaire aims to determine the current level of reading


proficiency of Grade 6 pupils of Dr. Domingo G. Abcede Elementary School, and
learn about their attitude towards reading, and the perceived factors affecting
their reading proficiency.

As a respondent of the survey, you are highly encouraged to answer the


items completely with honesty and sincerity.

Note: The surveyors/researchers will guide the respondents in answering each


item. Put a check (✓) on each item that corresponds to the respondent’s answer.

I. PUPIL’S READING PROFICIENCY 4 3 2 1
1. I can recognize the letters of the alphabet.
2. I am able to produce the sound of the letters of each
alphabet.
3. I am able to combine the sounds each letter.
4. I can blend the sound of letters to produce words.
5. I can read and pronounce the words correctly.
6. I am able to read aloud with fluency.
7. I can understand the meaning of the words that I
have read.
8. I can understand the meaning of the phrases, texts or
paragraphs that I have read.
9. I am able to infer meaning from the text that I have
read.
