The Level II BSN students from Calamba Doctors' College had their second hospital exposure at Dr. Jose P. Rizal Memorial Hospital from 2-10 PM. They signed in, prepared, and were assigned new patient groups. They introduced themselves to patients, took assessments and vitals from 2:30-5 PM. From 5-6:30 PM they reviewed patient files and from 6:30-7 PM had dinner. By 8 PM they had finished vitals and file reviews to prepare for their post-conference, which was postponed after two students successfully inserted a catheter under supervision.

NARRATIVE REPORT

The Level II Bachelor of Science in Nursing Students, group 1 of Calamba Doctors

College had their second hospital duty exposure for the second semester at Dr. Jose P. Rizal

Memorial Hospital last February 6, 2023, Monday from 2:00 pm to 10:00 pm shift under the care

of their clinical instructor Mr. Rene Mundin R.N., M.A.N.

We had to sign our attendance form with the date, time, and signature when we arrived at

the hospital. Because we are educated to be on time, early, or punctual, we arrived at the hospital

earlier than expected. Before beginning our activity, we are requested to prepare and arrange our

belongings. The grouping for RLE duty was changed and some of our old members were

assigned to another group and were replaced with new members. Before we start our duty, we

had to do a short orientation for our new members since it was their first time joining our group.

At the nursing station, we made the decision to form a group of two or three people to

help the patients who were assigned to us, one of our classmates identified the patients who

would be allocated to each group. We began our endorsements and rounds at precisely 2:30 pm.

As soon as we entered the room, we introduced ourselves to our designated patients and start our

Nursing-Patient Interaction (NPI) by requesting the patient's name for purposes of verification.

We immediately began assessment or evaluation, history taking (also known as an interview),

and vital sign taking, which lasts till 5:00 p.m.

In addition, from 5:00 pm to 6:30 pm, we examine our patient's files after our nurse-

patient interaction. We write down some crucial information for our tickler or notepad, such as

the diagnosis, primary complaint, medical background, laboratory results, etc. As scheduled, we

have our dinner from 6:30 pm to 7:00 pm. By 8:00 p.m., we had finished studying the patient

records and had taken the patient's vital signs, logged them on the vital sign monitors, and looked

for any changes or abnormalities. We then continued to review the patient files as we did earlier

to prepare for our post-conference.

Today’s duty was full of learning and experience. Some of us were able to change and

regulate patients' IV fluid, checked if the patient's NGT was in place through auscultating their

stomach while giving air on the tube, assist, and observe how ECG was done. The highlights of

today's duty are when two of our group members were able to insert a catheter into the newly

admitted patient. We determined the two members who will perform it through a toss. All of us

were excited and willing to perform the procedure because it will be our first time and the

experience it will give us can help us in the future. Sadly, not all of us can perform the procedure

but we are all happy and proud of the two members who did it. After we toss, two of our

classmates volunteered to assist Sir. Rene Mundin at the room of the patient. The two students

were relieved and frightened at the same time after successfully inserting the catheter into the

patient. After that procedure, our clinical instructor Sir Rene decided to postpone our post-

conference and he decided to end the shift duty for that day.
