This has reference to the present medical condition of the undersigned, which he
has been infected with Sexual Transmitted Diseases (STD).

In this regard, the undersigned would like to request permission that he will be
permitted to monetize his unused leave credits for he will use the proceeds for
medical purposes, specifically for his medication of the above-mentioned diseases.

This letter will serve as my manifestation or intention to monetize my unused leave


credits.
