MENTAL HEALTH GLOBAL ACTION PROGRAM (mhGAP)
I. Case Scenario
B. Lito 45 years of age is a janitor working in a Chinese restaurant was laid off from work since there is
a sudden decrease of costumers due to pandemic the restaurant closed. A week after, his wife and 2
children were affected by Covid 19 virus disease and need to be confined in the hospital. His wife and
children died and all his savings were lost, he was severely affected and suffer from severe depression.
IV. Discussion
The group can answer it in form of discussion or a role play.
1. If you are the community health nurse how can you help Lito recover from severe depression?
Nursing Intervention
Allow the patient to recognize and express feelings and determine the connection between the feelings
and the event
Encourage patient to do as much as possible for self. The patient may need more direction with
activities of daily living if depression is severe.
Encourage the patient to increase self-esteem by being more compassionate towards himself.
Promote positive health behaviors, including medication compliance and healthy lifestyle choices.
Advise the patient to see a psychiatrist to be monitored and take medicines if needed or talk to someone
else that he truly trust.
Encourage him to have enough sleep and take breaks if needed.
Be active by getting regular exercise and on the community by means of socializing with other groups
or people.
Advise him to make "to do list" for his every day
In this case, since Lito is suffering from severe depression, he may not have the strength to talk
to someone, because he’s not functioning well due to lack of courage to move forward in life.
Therefore, it should be his relatives who take action on his behalf. His relatives may post his
story of what happened; thus, the government will be aware as well as other people. His
relatives may also write a formal letter describing what Lito is going through, then pass it in
their local government to get some financial or medical assistance. And if the government
noticed their concern, they will send social worker to visit him in his respective home.
Then, the social worker should talk to Lito in a well-mannered way to have a good and smooth
conversation. This will help Lito to calm his mind despite of the depression he’s suffering for.
After this, he will get the help he needed the most.
3. What are services that government can offer for free to Lito and people like him?

In the Philippines the DOH participate in the WHO program that focuses on mental health. They
published a law about it and this is the REPUBLIC ACT No. 11036 An Act Establishing a National
Mental Health Policy for the Purpose of Enhancing the Delivery of Integrated Mental Health Services,
Promoting and Protecting the Rights of Persons Utilizing Psychosocial Health Services, Appropriating
Funds Therefor and Other Purposes.
During this time of crisis, PAP offer different services for the mental health of people. They implement
strategies for promotion of quality care and prevention in mental health disorders. One of the program
they promote is the online counseling which are available anywhere, can be easily and free to access.
Assistance to Individuals/ Families in Crisis Situations
The Assistance to Individuals in Crisis Situations (AICS) is one of protective services of DSWD that
started in March 2020. It is a social safety net that supports the recovery of individuals and families
(poor, marginalized, and vulnerable or disadvantaged individuals) from abrupt crisis such as illness or
death of a family member, and other crisis situations.
The Crisis Intervention unit, which also operates through the Department of Social Welfare and
Development Teams in different regions, is a special unit that immediately responds to the case of
individuals and families in crisis situations. Integrated services such as immediate rescue and
protection, provision of direct financial and material assistance, augmentation during disaster and
referrals for immediate medical, legal, psycho-social services are provided the unit. Filipinos who would
like to apply for AICS can receive medical assistance, burial assistance and educational assistance.
